摘  要:目的评价能谱CT物质分离技术测定骨密度(BMD)的准确性及与定量CT(QCT)测定BMD的一致性.方法对包含3个椎体(L1,L2,L3)的欧洲脊椎体模(ESP)进行不同辐射剂量的能谱CT成像.此外,对16只5月龄的新西兰雌兔进行QCT和能谱CT成像,分别测量每只雌兔L3及L4椎体的BMD值.结果L1、L2、L3椎体在不同辐射剂量下基物质值均无统计学差异(P>0.05).L1和L2椎体的羟基磷灰石(HAP)(水)值与实际值相似.L3椎体的相对误差(RE)的四分位数和中位数均<6%.L1、L2和L3椎体的HAP(脂肪)值和钙(水)值与实际值有显著差异.能谱扫描与QCT扫描测得BMD值呈高度线性相关.Bland-Altman图提示二者测量一致性良好.结论能谱CT测量BMD有较高的准确性,与QCT测定BMD一致性良好,在临床中基于能谱CT的腰椎BMD测量是可行的.Objective To evaluate the accuracy of energy spectral CT material separation technology in determining bone mineral density(BMD),and to examine its consistency with quantitative computed tomography(QCT)BMD measurements.Methods The European spine phantom(ESP),which contains three vertebral bodies(Ll,L2,L3)underwent energy spectrum imaging with different radiation doses.16 New Zealand female rabbits aged 5 months underwent QCT and energy spectral CT imaging to measure the BMD values of their L3 and L4 vertebral bodies.Results There were no significant differences in basis pair values of the Ll,L2,or L3 vertebrae under different radiation doses(P>0.05).The hydroxyapatite(HAP)(water)values of the L1 and L2 vertebrae were similar to the actual values.The quartile and median of relative error(RE)of L3 vertebra were both less than 6%.The HAP(fat)values and the calcium(water)values of the Ll,L2,and L3 vertebrae were significantly different from the actual values.The BMD measured with the energy spectrum scan and that measured with the QCT scan had a high linear correlation.The Bland-Altman plot showed that BMD measured by the two methods were good agreement.Conclusion Energy spectrum CT is accurate when measuring the BMD and it is highly correlated with the QCT.It is feasible to measure lumbar vertebra BMD based on energy spectrum CT in clinical practice.
