摘 要:目的 对云南省首例输入性寨卡病毒病病例进行描述性流行病学分析以及实验室检测结果分析,为防控输入性传染病疫情提供参考。方法 对2019年云南省首个输入性寨卡病毒病病例进行流行病学调查与处置,并对病例的血清和尿液标本进行核酸检测;同时对病例标本进行高通量测序,测序结果在CLC Genomics Workbench软件上进行分析拼接,最后利用Mega软件进行病毒基因分子特征分析和系统进化树构建及同源性分析。结果 该病例回国前在缅甸有发热、头痛和眼眶疼痛的症状,入境后核酸检测结果显示寨卡病毒阳性。系统发育树显示该云南寨卡病毒株属亚洲系东南亚流行分支,基因同源性分析表明该寨卡病毒株与东南亚、太平洋和美国寨卡病毒株的同源性在97.3%~99.1%。该寨卡病毒具有139N、982V、2634M的基因分子特征。结论 2019年7月云南省首例输入性寨卡病毒病病例的寨卡病毒株属于亚洲系东南亚分支。该病例于缅甸感染,其流行病学调查结果与实验室诊断结果一致。云南与东南亚接壤,面临着巨大的新发再发传染病输入风险,全省应加强边境传染病监测与检测工作。Objective To describe the clinical manifestations and epidemiological history of the first imported case with Zika virus ZIKV infection in Yunnan province, and to discuss the laboratory diagnosis and genetic evolution. Methods An epidemiological investigation of the first imported case with Zika virus(ZIKV) infection was conducted, and real-time RT-PCR test was performed for pathogen confirmation.The whole genome sequence was obtained through next-generation sequencing(NGS).MEGA 7.0 software was used for phylogenetic tree construction. Results The case had symptoms of fever, headache, and orbital pain in Myanmar before returning home in Yunnan.Both serum and urine specimens were ZIKV positive analyzed by real time RT-PCR.Gene homology analysis showed that the homology between Yunnan strain and Southeast Asia, Pacific and American strains ranged from 97.3% to 99.1%.The imported strain showed unique molecular features of 139 N, 982 V and 2634 M. Conclusions The ZIKV strain of Yunnan/01/2019 belongs to the Asian family, which agrees with the epidemiological investigation outcome.This case does contract ZIKV in Myanmar.Yunnan,as a border of Southeast Asia,faces a high risk of imported infectious diseases.Therefore,infectious disease prevention and control should be strengthened.
