摘　　要：目的探讨多睾症合并隐睾症诊治策略。方法分析2021年7月贵州省人民医院收治的1例多睾症合并隐睾症患者的临床资料、治疗方式及随访情况,结合文献复习探讨其诊治策略。结果患者第1次就诊时被误诊为普通腹股沟型隐睾并行睾丸下降固定术,术后1年复查发现左侧阴囊内原下降睾丸萎缩,盆腔内隐睾影。第2次手术探查后临床诊断考虑多睾症合并隐睾症,并行保留多睾下降固定术,术后6个月复查提示左侧萎缩睾丸、少弱精子症。结论多睾症较罕见,彩超对体积较小的睾丸或位于腹腔的睾丸不易查出,易漏诊误诊。对于具有潜在生殖功能且具有生殖要求的多睾症患者,可行保留多生睾丸下降固定术。隐睾症常伴有少弱精子症,术后常见并发症为睾丸萎缩,合并多睾症时癌变率相对更高,需定期随诊复查。Objective To investigate the diagnosis and treatment strategies of polyorchidism with cryptorchidism.Methods This paper analyzed the clinical data,treatment methods and follow-up of a polyorchidism with cryptorchidism patient admitted to Guizhou Province People′s Hospital in July 2021,and discussed the diagnosis and treatment strategy combined with literature review.Results At the first visit,the patient was misdiagnosed as common inguinal cryptorchidism and was performed testicular descent fixation.1 year after the operation,the reexamination found atrophic testis in the left scrotum and cryptorchidism shadow in pelvic cavity.After the second surgical exploration,polyorchidism with cryptorchidism was considered,and keeping multiple testes during testicular descent fixation.6 months after the secondary operation,the reexamination suggested left atrophic testis and oligospermia.Conclusions Polyorchidism is rare.Color ultrasonography is not easy to detect when testicular is small or located in the abdominal cavity,and it is easy to be missed and misdiagnosed.For patients with potential reproductive function and reproductive requirements,the retention of multiple testicular descent fixation is feasible.Cryptorchidism is often accompanied by oligospermia.The common postoperative complication is testicular atrophy.The cancer rate is relatively higher when combined with polyteschidism,which requires regular follow-up examination.
