摘　　要：目的探讨肥胖人群的代谢性内毒素血症与精子DNA碎片指数之间是否存在相关性。方法选取2021年1月至6月在甘肃省妇幼保健院就诊的138例男性不育症患者,检测其血清人连蛋白(Zonulin)、血清脂多糖结合蛋白(LBP)、人精液弹性蛋白酶(Elastase)、精液8-羟基脱氧鸟苷(8OHdG)、精子DNA碎片指数(DFI)等。采用Pearson相关系数分析各指标间的相关性。结果Zonulin与LBP之间r=0.352,P=0.000,呈正相关;LBP与8OHdG之间r=0.540,P=0.000,呈正相关;8OHdG与体重指数(BMI)之间r=0.562,P=0.000,呈正相关;LBP与DFI之间r=0.524,P=0.000,呈正相关;DFI与Elastase之间r=-0.035,P=0.680,无相关性;LBP与Elastase之间r=-0.008,P=0.923,无相关性。结论肥胖可诱发肠道细菌易位,易位的细菌可诱导代谢性内毒素血症发生,内毒素水平的升高可造成全身炎症反应,致使氧化应激出现,而氧化应激可造成白细胞释放活性氧,活性氧会造成精子DNA损伤。Objective To investigate whether there is a correlation between metabolic endotoxemia and sperm DNA fragmentation rate in obese people.Methods 138 male infertility patients in Gansu Provincial Maternity and Child-care Hospital from January to June 2021 were selected.The serum Zonulin,serum lipopolysaccharide binding protein(LBP),human semen Elastase,semen 8-hydroxy-2′-deoxyguanosine(8OHdG)and sperm DNA fragmentation index(DFI)were measured respectively.Pearson method was used to analyze the correlation between them.Results The correlation coefficient between Zonulin and LBP was r=0.352,P=0.000,which was positively correlated.The correlation coefficient between LBP and 8OHdG was r=0.540,P=0.000,which was positively correlated.The correlation coefficient between 8OHdG and body mass index(BMI)was r=0.562,P=0.000,which was positively correlated.The correlation coefficient between LBP and DFI was r=0.524,P=0.000,which was positively correlated.The correlation coefficient between DFI and Elastase was r=-0.035,P=0.680,and there was no correlation.The correlation coefficient between LBP and Elastase was r=-0.008,P=0.923,and there was no correlation.Conclusions Obesity can induce intestinal bacterial translocation,and translocated bacteria can induce metabolic endotoxemia.The increase of endotoxin level can cause systemic inflammatory reaction and oxidative stress.Oxidative stress can cause leukocytes to release reactive oxygen species,which can cause sperm DNA damage.
