多西他赛治疗中晚期前列腺癌提升患者性生活质量的研究及血清学指标水平分析

摘　　要：目的分析多西他赛治疗中晚期前列腺癌对患者性生活质量及血清学指标水平的影响。方法选取2019年1月至2020年12月金华市中心医院收治的126例中晚期前列腺癌患者作为研究对象。随机分为比卡鲁胺组和联合多西他赛组,各63例。比卡鲁胺组采用比卡鲁胺片进行治疗,联合多西他赛组在比卡鲁胺组的基础上联合多西他赛进行治疗,两组均治疗1年。统计两组治疗1年后的疗效,比较两组治疗前和治疗1年后血清细胞因子水平及性生活质量。结果治疗1年后,联合多西他赛组总有效率显著高于比卡鲁胺组,差异具有统计学意义(P<0.05)。与治疗前比较,治疗1年后两组血清胰岛素样生长因子-1(IGF-1)、腺激肽释放酶2(hK2)、前列腺特异性抗原(PSA)、血管内皮生长因子(VEGF)水平降低,且联合多西他赛组低于比卡鲁胺组,差异具有统计学意义(P<0.05)。与治疗前比较,治疗1年后两组性生活质量、性愉悦、性趣评分降低,但联合多西他赛组高于比卡鲁胺组,差异具有统计学意义(P<0.05);两组治疗相关症状、肠道症状评分升高,尿路症状评分降低(P<0.05),但组间比较差异无统计学意义(P>0.05)。结论多西他赛治疗中晚期前列腺癌患者,可有效改善患者血清细胞因子水平、性生活质量及功能状态,具有较好的治疗效果。Objective To analyze the effect of docetaxel in the treatment of advanced prostate cancer on the quality of sexual life and the level of serum indexes.Methods From January 2019 to December 2020,126 patients with advanced prostate cancer in Jinhua Municipal Central Hospital were taken as research objects,and they were divided into the bikalutamine group(63 cases)and the combined docetaxel group(63 cases)by random number table method.The bikalutamine group was treated with bicalutamide tablets,and the combined docetaxel group was treated with docetaxel on the basis of the bikalutamine group.Both groups were treated for 1 year.After 1 year of treatment,the curative effect in the two groups were counted,and the levels of serum cytokines,the quality of sexual life,functional status before and after 12 months of treatment were compared.Results After 1 year of treatment,the total effective rate of the combined docetaxel group was significantly higher than that of the bikalutamine group(P<0.05).Compared with before treatment,after 1 year of treatment,the levels of serum insulin-like growth factor-1(IGF-1),human kallikrein 2(hK2),prostate specific antigen(PSA),vascular endothelial growth factor(VEGF)in the combined docetaxel group were lower than those in the bikalutamine group(P<0.05);the scores of the quality of sexual life,sexual pleasure and sexual interest in the two groups decreased,and those in the combined docetaxel group were higher than those in the bikalutamine group(P<0.05).The scores of treatment-related symptoms and intestinal symptoms in the two groups increased,while the scores of urinary tract symptoms decreased(P<0.05),there was no significant difference between the two groups(P>0.05).Conclusions Docetaxel could effectively improve the level of serum cytokines,quality of sexual life and functional status in patients with advanced prostate cancer,and has a therapeutic effect.
