摘  要:目的探究微小RNA-200c(miR-200c)联合人附睾上皮分泌蛋白4(HE4)、糖类抗原125(CA125)诊断上皮性卵巢癌(EOC)的价值。方法将2018年1月至2020年1月重庆市黔江中心医院诊治的54例EOC患者作为EOC组,54例卵巢良性疾病患者作为卵巢良性疾病组,54例健康女性作为正常组。检测所有受试者血清miR-200c、HE4、CA125水平;以Pearson法分析EOC组血清miR-200c水平与HE4、CA125水平的相关性;分析EOC组血清miR-200c表达水平与临床病理特征的关系;采用受试者工作特征(ROC)曲线评价血清miR-200c、HE4、CA125水平对EOC的诊断价值。结果EOC组血清miR-200c表达水平明显低于正常组、卵巢良性疾病组(P<0.05),HE4、CA125水平明显高于正常组、卵巢良性疾病组(P<0.05);卵巢良性疾病组血清CA125水平明显高于正常组(P<0.05)。EOC组血清miR-200c表达水平与HE4、CA125水平均呈负相关(P<0.05);EOC组血清miR-200c表达水平与肿瘤临床分期、淋巴结转移相关(P<0.05);miR-200c是影响EOC发生的保护因素,HE4、CA125是影响EOC发生的危险因素(P<0.05);血清miR-200c联合HE4、CA125诊断EOC的曲线下面积(AUC)为0.951,其灵敏度、特异度分别为88.9%、88.9%;血清miR-200c联合HE4、CA125的诊断效能高于miR-200c、HE4、CA125单独诊断。结论EOC患者血清miR-200c表达下调,HE4、CA125水平上调,三者可能在EOC发展中起作用,miR-200c联合HE4、CA125可更好地筛查EOC患者。Objective To explore the value of microRNA-200c(miR-200c)combined with human sapiens epididymis specific protein 4(HE4)and carbohydrate antigen 125(CA125)in the diagnosis of epithelial ovarian cancer(EOC).Methods From January 2018 to January 2020,54 EOC patients treated in Qianjiang Central Hospital of Chongqing were selected as EOC group,54 patients with benign ovarian disease as ovarian benign disease group and 54 healthy women as normal group.The levels of serum miR-200c,HE4 and CA125 were detected in all subjects;Pearson method was used to analyze the correlation between serum miR-200c level and HE4,CA125 in EOC group;the relationship between serum miR-200c expression and clinicopathological features in EOC group was analyzed;the diagnostic value of serum miR-200c,HE4 and CA125 levels for EOC was evaluated by receiver operating characteristic(ROC)curve.Results The expression of serum miR-200c in the EOC group was significantly lower than that in the normal group and the benign ovarian disease group(P<0.05),the levels of HE4 and CA125 were significantly higher than those in the normal group and the benign ovarian disease group(P<0.05);the level of serum CA125 in the benign ovarian disease group was significantly higher than that in the normal group(P<0.05).In the EOC group,the expression level of miR-200c was negatively correlated with the levels of HE4 and CA125(P<0.05);the expression of miR-200c was correlated with clinical stage and lymph node metastasis(P<0.05);miR-200c was the protective factor affecting the occurrence of EOC,and HE4 and CA125 were the risk factors affecting the occurrence of EOC(P<0.05);the area under the curve(AUC)of serum miR-200c combined with HE4 and CA125 for the diagnosis of EOC was 0.951,and its sensitivity and specificity were 88.9%and 88.9%,respectively.The diagnostic efficacy of serum miR-200c combined with HE4 and CA125 was higher than that of miR-200c,HE4 and CA125 alone.Conclusions The expression of serum miR-200c is down-regulated and the levels of HE4 and CA125 are up-regu
