摘　　要：目的研究血清凋亡抑制蛋白抗体(Livin)、张力蛋白同源物(PTEN)、多药耐药基因1(MDR1)的表达与卵巢癌患者临床病理特征的关系及对化疗耐药、生存期的预测。方法选取2018年1月至2022年1月苏州市中西医结合医院诊治的120例卵巢癌患者作为研究对象(观察组),进行前瞻性研究。另选取同期进行健康体检120例健康人设为对照组。比较两组及不同病理特征患者的Livin、PTEN、MDR1水平,分析Livin、PTEN、MDR1的表达对化疗耐药、生存期的预测价值。结果观察组Livin(t=17.000,P=0.000)、PTEN(t=7.172,P=0.000)、MDR1(t=43.206,P=0.000)水平显著高于对照组;不同肿瘤直径、脉管侵犯、TNM分期、淋巴结转移、耐药情况及生存情况患者的Livin、PTEN、MDR1水平比较,差异具有统计学意义(P<0.05);相关性分析结果显示,肿瘤直径、脉管侵犯、TNM分期、淋巴结转移、耐药情况及生存情况与Livin、PTEN、MDR1水平呈正相关(P<0.05);Livin、PTEN、MDR1水平联合检测对于化疗耐药、生存期的诊断效能显著高于单独检测(P<0.05)。结论Livin、PTEN、MDR1的表达与卵巢癌患者临床病理特征有显著相关性,且三者联合检测对于化疗耐药、生存期具有更显著的预测价值。Objective To study the relationship between the expression of Livin,recombinant phosphatase and tensin homolog(PTEN)and multidrug resistance 1(MDR1)and the clinicopathological characteristics of ovarian cancer,and to predict the chemotherapy resistance and survival time.Methods A total of 120 patients with ovarian cancer diagnosed and treated in Suzhou Integrated Traditional Chinese and Western Medicine Hospital from January 2018 to January 2022 were selected as subjects(observation group)for prospective study.Another 120 healthy subjects who underwent physical examination during the same period were selected as the control group.The levels of Livin,PTEN and MDR1 were compared between the two groups and patients with different pathological characteristics,and the predictive value of the expressions of Livin,PTEN and MDR1 on chemotherapy resistance and survival was analyzed.Results The levels of Livin(t=17.000,P=0.000),PTEN(t=7.172,P=0.000)and MDR1(t=43.206,P=0.000)in the observation group were significantly higher than those in the control group.There were significant differences in Livin,PTEN and MDR1 levels among patients with different tumor diameter,vascular invasion,TNM stage,lymph node metastasis,drug resistance and survival(P<0.05).Correlation analysis showed that tumor diameter,vascular invasion,TNM stage,lymph node metastasis,drug resistance and survival were positively correlated with Livin,PTEN and MDR1 levels.The diagnostic efficacy of combined examination of Livin,PTEN and MDR1 for chemotherapy resistance and survival was significantly higher than that of single detection.Conclusions The expressions of Livin,PTEN and MDR1 were significantly correlated with the clinicopathological features of ovarian cancer patients.The combined detection of Livin,PTEN and MDR1 had significant predictive value for chemotherapy resistance and survival.
