艾滋病患者并发心血管系统损害的影响因素分析

摘　　要：目的分析艾滋病患者并发心血管系统损害的影响因素,旨在为艾滋病患者心血管系统损害的防治提供依据。方法回顾性分析2018年5月至2019年10月武汉市金银潭医院收治的117例艾滋病患者的病例资料。根据患者是否发生心血管损害分为两组。将未发生心血管系统损害70例患者纳入未发生组,将发生心血管系统损害47例患者纳入发生组。设计一般情况调查表,仔细查阅患者病历资料,采用单因素与多因素分析诱发艾滋病患者心血管系统损害的影响因素。结果单因素分析结果显示,血常规异常、丙氨酸氨基转移酶(ALT)升高、血清蛋白异常、肺部疾病、胆道损害均可能是艾滋病患者并发心血管系统损害的影响因素(P<0.05);对单因素明确为影响因素的变量赋值,多因素Logistic回归分析结果显示,血常规异常、ALT升高、血清蛋白异常、肺部疾病、胆道损害均是艾滋病患者并发心血管系统损害的影响因素(OR>1,P<0.05),其中ALT升高带来的影响最明显,可能作为独立影响因素存在。结论艾滋病患者存在较高的心血管系统损害风险,可能与患者血常规异常、血清蛋白异常、ALT显著增加、肺部疾病、胆道损害等因素相关。Objective To analyze the influencing factors of cardiovascular system damage in patients with acquired immune deficiency syndrome(AIDS),and to provide a basis for the prevention and treatment of cardiovascular system damage in AIDS patients.Methods The data of 117 AIDS patients admitted to Wuhan Jinyintan Hospital from May 2018 to October 2019 were retrospectively analyzed.Patients were divided into two groups according to whether they had cardiovascular system damage.70 patients with no cardiovascular system damage were included in the without cardiovascular system damage group and 47 patients with cardiovascular system damage were included in the with cardiovascular system damage group.The questionnaire of general condition was designed,the medical records of patients were carefully consulted,and the influencing factors of cardiovascular system damage in AIDS patients were analyzed by univariate and multivariate analysis.Results Univariate analysis showed that abnormal blood routine,elevated alanine aminotransferase(ALT),abnormal serum protein,pulmonary disease and biliary tract damage may be the influencing factors of cardiovascular system damage in AIDS patients(P<0.05).After assigning values to variables whose single factor is clearly an influencing factor,multivariate Logistic regression analysis showed that abnormal blood routine,elevated ALT,abnormal serum protein,pulmonary disease,and biliary tract damage were all influential factors of cardiovascular system damage in AIDS patients(OR>1,P<0.05),among which ALT≥40IU/L brought the most obvious influence,and may exist as an independent influencing factor.Conclusions AIDS patients have a higher risk of cardiovascular system damage,which may be related to abnormal blood routine,serum protein abnormalities,significant elevation of ALT,pulmonary disease,biliary tract damage of patients.
