摘  要:目的观察苗药五藤膏外敷对膝骨关节炎(KOA)兔模型关节软骨SMAD1及血清Jagged1、Jagged2表达的影响,探讨苗药五藤膏治疗KOA的可能机制。方法选取健康新西兰大白兔72只,随机分为空白组、模型组、巴布膏组及苗药五藤膏低、中、高剂量组,每组12只,除空白组外,其余各组均采用木瓜蛋白酶膝关节内注射方法建立兔KOA模型,给药4 w后处死动物解剖患膝关节软骨进行切片、苏木素-伊红(HE)染色及病理学Mankin评分,采用Western印迹法检测SMAD1蛋白表达水平,酶联免疫吸附试验(ELISA)检测血清Jagged1、Jagged2表达水平。结果与空白组相比,模型组兔软骨Mankin评分和血清Jagged1、Jagged2表达水平明显升高,SMAD1蛋白表达水平明显降低(P<0.05);与模型组相比,治疗组Mankin评分和Jagged1、Jagged2表达水平明显降低(P<0.05),SMAD1蛋白表达水平明显升高,其中除苗药五藤膏低剂量组外,其余各组均有统计学意义(P<0.05);治疗组内比较,Mankin评分从高到低依次为苗药五藤膏低、中剂量组、巴布膏组及苗药五藤膏高剂量组,差异有统计学意义(P<0.05);SMAD1蛋白表达水平从低到高依次为苗药五藤膏低、中、高剂量组及巴布膏组,其中巴布膏组与苗药五藤膏低、中剂量组相比,差异有统计学意义(P<0.05);Jagged1、Jagged2表达水平从高到低依次为苗药五藤膏低、中、高剂量组及巴布膏组,其中巴布膏组Jagged1表达水平与苗药五藤膏各剂量组相比,差异有统计学意义(P<0.05),巴布膏组Jagged2表达水平与苗药五藤膏低、中剂量组相比,差异有统计学意义(P<0.05);苗药五藤膏组组内比较,SMAD1蛋白表达水平差异无统计学意义(P>0.05),苗药五藤膏低剂量组Jagged1、Jagged2表达水平与中、高剂量组相比,差异有统计学意义(P<0.05)。结论苗药五藤膏可能通过促进KOA模型兔软骨细胞中SMAD1蛋白表达,抑制血清中Jagged1、Jagged2表达,延缓KOA的病程进展,从�Objective To observe the effect of external application of Miao medicine Wuteng ointment on the expression of SMAD1 in articular cartilage and serum Jagged1 and Jagged2 in the knee osteoarthritis(KOA)rabbit model,and to explore the possible mechanism of Miao medicine Wuteng ointment for KOA.Methods A batch of 72 healthy New Zealand white rabbits were selected and randomly divided into blank group,model group,Babu ointment group and Miaoyao Wuteng ointment low,medium and high dose groups,each with 12 rabbits,except for the blank group,the other groups were used to establish the rabbit KOA model by intra-knee injection of papain.After 4 weeks of administration,the animals were sacrificed to dissect the knee articular cartilage for sectioning,htoxylin eosin(HE)staining and pathological Mankin score.The expression level of SMAD1 protein was detected by Western blot,and the expression level of serum Jagged1 and Jagged2 were detected by enzyme linked immunosorbent assay(ELISA).Results Compared with the blank group,the rabbit cartilage Mankin score and serum Jagged1 and Jagged2 expression levels in the model group were significantly increased,while the SMAD1 protein expression level was significantly reduced(P<0.05);compared with the model group,the Mankin score and Jagged1 and Jagged2 expression levels in the treatment group were significantly reduced(P<0.05),and the expression level of SMAD1 protein was significantly increased.Except for the low-dose Wuteng ointment group,the rest of the groups had a statistically significant difference(P<0.05);the Mankin score in the treatment group ranged from high to low order is Wuteng ointment low,medium dose group,Babu ointment group and Wuteng ointment high dose group,the difference was statistically significant(P<0.05);the expression level of SMAD1 protein from low to high is Wuteng ointment low,medium and high dose group and Babu ointment group,and the differences between Babu ointment group and Wuteng ointment low and medium dose groups were statistically significant(P<0.05)
