针刺下关穴与胸锁乳突肌起止点对PFP伴耳颞神经痛的临床疗效

摘　　要：目的:探究临床治疗周围性面瘫(PFP)伴耳颞神经痛采用针刺下关穴、胸锁乳突肌起止点疗效。方法:选取本院2021年1月前到院就诊的45例PFP伴耳颞神经痛患者为对照组,选取2021年1月至2021年12月到院就诊的45例PFP伴耳颞神经痛患者为观察组。对照组采用传统针刺结合针刺下关穴治疗,观察组采用传统针刺结合针刺下关穴与胸锁乳突肌起止点治疗,观察两组治疗前后面神经功能分级、面部残疾指数、神经传导功能、面瘫康复情况及耳颞神经痛情况。结果:治疗后两组面神经功能分级、面部残疾指数、神经传导功能、面瘫症状及耳颞神经痛情况均显著改善(P<0.05),治疗后观察组面神经功能分级显著优于对照组,躯体功能评分显著高于对照组(P<0.05),社会功能评分显著低于观察组(P<0.05),M波波幅显著高于对照组(P<0.05),运动潜伏期显著低于对照组(P<0.05),面瘫康复情况Portmann评分显著高于对照组(P<0.05),耳颞神经痛视觉模拟(VAS)评分显著低于对照组(P<0.05)。结论:在传统针刺治疗的基础上针刺下关穴与胸锁乳突肌起止点能够显著改善PFP伴耳颞神经痛患者的面部微循环及神经传导功能,有效缓解面瘫症状,减轻耳颞神经痛,加速康复。Objective:To explore the clinical effect of acupuncture at Xiaguan point and starting and ending points of sternocleidomastoid muscle on peripheral facial paralysis(PFP)with ototemporal neuralgia.Methods:45 cases of PFP with ototemporal neuralgia who came to the hospital before January 2021 were selected as the control group,and 45 cases of PFP with ototemporal neuralgia who came to the hospital from January 2021 to December 2021 were selected as the observation group.The control group was treated with traditional acupuncture combined with acupuncture at Xiaguan point,while the observation group was treated with traditional acupuncture combined with acupuncture at Xiaguan point and the starting and ending points of sternocleidomastoid muscle.The functional grading of the front and back nerves,facial disability index,nerve conduction function,facial paralysis rehabilitation and ototemporal neuralgia were observed in the two groups.Results:After treatment,the facial nerve function grade,facial disability index,nerve conduction function,facial paralysis symptoms and ototemporal neuralgia in the two groups were significantly improved(P<0.05).After treatment,the facial nerve function grade in the observation group was significantly better than that in the control group,the somatic function score was significantly higher than that in the control group(P<0.05),the social function score was significantly lower than that in the observation group(P<0.05),and the amplitude of M-wave was significantly higher than that in the control group(P<0.05),The motor latency was significantly lower than that of the control group(P<0.05),the Portmann score of facial paralysis rehabilitation was significantly higher than that of the control group(P<0.05),and the visual analogue of ototemporal neuralgia(VAS)score was significantly lower than that of the control group(P<0.05).Conclusion:On the basis of traditional acupuncture treatment,acupuncture at Xiaguan point and the starting and ending points of sternocleidomastoid muscle can signifi
