井穴放血联合针刺治疗紫杉类化疗药物相关周围神经病变的临床疗效观察

摘　　要：目的:观察井穴放血联合针刺治疗紫杉类化疗药物相关周围神经病变的临床疗效。方法:采用随机对照研究,纳入紫杉类化疗药物所致周围神经病变的患者,对照组予甲钴胺口服,治疗组在对照组治疗基础上,加用井穴放血联合针刺治疗,14天/疗程,2个疗程后,观察2组患者的疗效,CIPN分级、中医证候量表评分、生活质量评分(KPS)和神经传导速度的改善情况。结果:治疗组(30例)总有效率73.33%,高于对照组(30例)的43.33%(P<0.05);治疗组治疗后的中医证候积分低于对照组(P<0.05),KPS评分高于对照组(P<0.05),CIPN分级、神经传导速度的改善情况优于对照组(P<0.05)。结论:井穴放血联合针刺能明显改善紫杉类化疗药物所致周围神经病变患者的临床症状,降低中医证候积分,改善周围神经毒性分级、生活质量评分和神经传导速度,提高甲钴胺治疗CIPN的临床疗效。Objective:To observe the clinical effect of Bloodletting in jing points combined with acupuncture in the treatment of taxane chemotherapy-related peripheral neuropathy.Method:A randomized controlled study was conducted,Patients received chemotherapy containing taxanes and developed peripheral neurotoxicity were selected.The control group was treated with mecobalamin orally.On the basis of the control group,the treatment group was treated with well-point bloodletting combined with acupuncture.14 days were a course of treatment.After two courses of treatment,the therapeutic effect,CIPN classification,TCM syndrome scale score,quality of life score(KPS)and improvement of nerve conduction velocity were observed in the two groups.Results:The total effective rate of the treatment group was 73.33%,which was higher than 43.33%of the control group(P<0.05).After treatment,the TCM symptom score of the treatment group was lower than that of the control group(P<0.05),and the KPS score was higher than that of the control group(P<0.05),CIPN classification and improvement of nerve conduction velocity were better than those in the control group(P<0.05).Conclusion:Acupoint bloodletting combined with acupuncture can significantly improve the clinical symptoms of patients with peripheral neuropathy caused by taxane chemotherapy drugs,Reduce TCM syndrome score,improve peripheral nerve toxicity grade,quality of life score and nerve conduction velocity,and improve the clinical efficacy of mecobalamin in the treatment of CIPN.
