基于腧穴选用规律探析针刺治疗持续植物状态

摘　　要：目的:运用数据挖掘技术探讨针刺治疗持续植物状态的选穴规律。方法:通过计算机检索中国知网数据库(CNKI)、万方数据库(Wangfang Data)和维普数据库(VIP)从创刊至2021年11月1日关于针刺治疗持续性植物状态的临床研究文献,按照纳入和排除标准筛选文献、提取数据,使用IBM SPSS 26.0和SPSS Modeler 18.0等软件进行聚类分析和关联规则分析。结果:共纳入30个针灸处方,共计穴位87个,累计使用频次388次,使用频次前8位的穴位分别为水沟、内关、三阴交、百会、涌泉、四神聪、合谷和神庭,经脉选择以督脉、足少阳胆经和手厥阴心包经上的穴位最多,穴位分布多集中在头颈项部、下肢部及上肢部,重视选用五输穴、交会穴等特定穴,关联规则分析显示前5位的穴位组合为内关-三阴交、内关-水沟、三阴交-内关、三阴交-水沟及内关-水沟-三阴交。聚类分析树状图得出形成4条核心穴组,分别为:神庭-本神-劳宫-风府-风池;内关-三阴交;涌泉-太冲-曲池;四神聪-合谷-足三里-水沟-百会。结论:针刺治疗持续植物状态采用近端取穴联合远端取穴的取穴原则,重视穴位配伍,所得出的穴对可为针刺治疗持续植物状态的临床应用提供参考。Objective:To explore the regularity of acupoint selection for persistent vegetative state with acupuncture using data mining technology.Methods:The Chinese National Knowledge Infrastructure(CNKI),Wangfang Data and VIP database(VIP)from the inception to November 1,2021for clinical research literature on acupuncture in the treatment of persistent vegetative state were searched by computer,and exclusion criteria were used to filter literature,extract data,and use IBM SPSS 26.0 and SPSS Modeler 18.0 software to perform cluster analysis and association rule analysis.Results:A total of 30 acupuncture and moxibustion prescriptions were included,with a total of 87 acupoints,and the cumulative frequency of use was 388.The top 8 acupuncture points were Shuigou,Neiguan,Sanyinjiao,Baihui,Yongquan,Sishencong,Hegu and Shenting.The most acupoints on the Du meridian,the gallbladder meridian of Foot Shaoyang and the pericardium meridian of hand Jueyin are selected.The distribution of acupoints is mostly concentrated in the head and neck,lower limbs and upper limbs,and emphasis is placed on the selection of specific points such as Wushu and Junhui.Association rule analysis showed that the top 5 acupoint combinations were Neiguan-Sanyinjiao,Neiguan-Shuigou,SanyinjiaoNeiguan,Sanyinjiao-Shuigou,and Neiguan-Shuigou-Sanyinjiao.Cluster analysis dendrogram shows the formation of 4 core acupoint groups,namely:Shenting-Benshen-Laogong-Fengfu-Fengchi;Neiguan-Sanyinjiao;Yongquan-Taichong-Quchi;Sishencong-Hegu-Zusanli-Shuigou-Baihui.Conclusion:Acupuncture treatment of persistent vegetative state adopts the principle of proximal acupoint selection combined with distal acupoint selection,and attaches great importance to the compatibility of acupoints.The obtained acupoint pairs can provide reference for the clinical application of acupuncture for persistent vegetative state.
