基于“四位一体”浅析痛泻要方治疗腹泻型-肠易激综合征

摘　　要：“四位一体”经方研究方法,即“方性”“方位”“方势”“方证”四维方面分别辨识经方的研究思路,通过立体化、多维度、多角度地分析经方,探讨痛泻要方治疗肠易激综合征(IBS)的机制即“一体”的整体治疗体现;多数学者认为肠易激综合征发病主要责之于肝、脾、肾,并与湿、瘀等有关,脾虚是其发病基础,肝郁是其发病关键,肝郁脾虚、肝脾不调是其重要病机。痛泻要方通过“性、位、势、证”四位发挥治疗肠易激综合征的作用。痛泻要方方性为甘、辛、苦味,属性温补,调和肝脾,脾健肝和,则痛泻自止;方位为大小肠且与肝脾胃密切相关;方势为向上、向内作用趋势,升浮向上以止泻,内敛以顾护津液以防滑脱;方证为脾虚肝郁证,现代多用于肠易激综合征、结肠炎、急慢性腹泻等。痛泻要方“一体”是由多药组成,又发挥着部分之和大于整体的作用;“一体”治疗肠易激综合征的药理机制包括降低内脏高敏性、调节肠道菌群、调节脑肠轴平衡等多因素相关,其治疗肠易激综合征的机制与临床疗效得到了临床和实验的验证。使用“四位一体”思想分析经方,使方证结合,对临床应用及经方的现代化发展具有重要意义。The research method of four-in-one Jing Fang,namely,the research ideas of Jing Fang were identified from the four-dimensional aspects of fang’s orientation,fang’s potential and Fang’s syndrome.Through three-dimensional,multi-dimensional and multi-angle analysis of Jing Fang,the mechanism of Tongxie Yaofang in the treatment of irritable bowel syndrome(IBS)was discussed,that is,the whole treatment of tongxie Yaofang.Most scholars believe that the occurrence of IRRITABLE bowel syndrome is mainly responsible for the liver,spleen and kidney,and related to dampness and blood stasis,spleen deficiency is the basis of its occurrence,liver depression is the key to its occurrence,liver depression,spleen deficiency,liver and spleen mismatch is the important pathogenesis of the treatment of irritable bowel syndrome Bitter taste,attribute warm tonic,harmonize liver and spleen,spleen and liver,the pain diarrhea since check;The orientation is large and small intestine and closely related to liver,spleen and stomach.The square potential is upward and inward,rising and floating upward to stop diarrhea,and collecting inward to protect body fluid to prevent slippage;Prescription syndrome is spleen deficiency and liver depression syndrome,which is mainly used for irritable bowel syndrome,colitis,acute and chronic diarrhea and other pain and diarrhea.Pharmacological mechanism of integrated treatment of irritable bowel syndrome include lowering visceral gao min sex regulating intestinal flora balance multiple factors such as related brain gut axis,its clinical curative effect and the mechanism of treatment of irritable bowel syndrome is a clinical and experimental verification of the use four integrated ideas analysis by the party,the party card combination,for clinical application and the party’s modernization is of great significance.
