自由体位分娩在胎头位置异常产妇中对总产程时间、出血量、剖宫产率及新生儿窒息发生率的影响

摘　　要：目的:研究探析自由体位分娩在胎头位置异常产妇中对总产程时间、出血量、剖宫产率及新生儿窒息发生率的影响。方法:回顾性选取我院2017年2月至2021年8月收治的56例分娩胎头位置出现异常的产妇作为研究对象,按分娩方式将其分为观察组和对照组,每组各28例。对照组予以传统体位分娩指导,观察组予以自由体位分娩指导。统计和对比两组产妇总产程、产后出血量、剖宫产率、新生儿窒息发生率、新生儿Apgar评分及产后护理总满意度。结果:观察组总产程为(216.03±7.15)min,短于对照组的(272.6±9.83)min;观察组产后出血量为(185.24±21.11)mL,少于对照组的(204.13±32.93)mL;观察组剖宫产率为7.14%,低于对照组的32.14%;观察组新生儿窒息发生率3.57%,低于对照组的21.43%;观察组新生儿评分为(9.89±0.11)分,高于对照组的(9.31±0.12)分;观察组的产后护理总满意度为100%,高于对照组的89.29%。结论:自由体位分娩能够有效帮助胎头位置异常产妇加快产程,降低产后出血量、剖宫产率和新生儿窒息发生率,提高产妇产后护理满意度。Objective:To investigate the influence of free position delivery on total labor time,blood loss,cesarean section rate and neonatal asphyxia rate in abnormal fetal head position.Methods:56 cases of parturient women with abnormal fetal head position in our hospital from February 2017 to August 2021 were retrospectively selected as the research object,and divided into observation group and control group according to the delivery mode,with 28 cases in each group.The control group was given traditional position delivery guidance,and the observation group was given free position delivery guidance.The total duration of labor,postpartum bleeding,cesarean section rate,incidence of neonatal asphyxia,Apgar score of newborn and total satisfaction of postpartum care were analyzed and compared between the two groups.Results:The total duration of labor in the observation group was(216.03±7.15)min,which was shorter than that in the control group(272.6±9.83)min.The postpartum blood loss in the observation group was(185.24±21.11)mL,which was less than that in the control group(204.13±32.93)mL.The rate of cesarean section in observation group was 7.14%,lower than 32.14%in control group.The incidence of neonatal asphyxia in the observation group was 3.57%,lower than 21.43%in the control group.The score of the observation group was(9.89±0.11),which was higher than that of the control group(9.31±0.12).The total satisfaction of postnatal care in the observation group was 100%,higher than that in the control group(89.29%).Conclusion:Free position delivery can effectively help women with abnormal fetal head position to speed up the labor process,reduce postpartum bleeding,cesarean section rate and neonatal asphyxia rate,and improve postpartum nursing satisfaction.
