营养支持小组对新生儿重症监护病房早产儿喂养及营养状况的影响

摘　　要：目的:探讨营养支持小组(NST)对新生儿重症监护病房(NICU)早产儿喂养及营养状况的影响。方法:采用前瞻性随机试验方法,选择2018年3月至2019年5月韶关市第一人民医院NICU收治的48例由新生儿科医师独立查房的早产儿作为对照组,选择2019年6月至2020年5月韶关市第一人民医院NICU收治的46例由NST查房的早产儿作为观察组。记录两组喂养情况、营养状况[白蛋白(ALB)、前白蛋白(PA)]、并发症发生率。结果:干预前,两组ALB、PA水平相比,差异无统计学意义(P>0.05);干预2周后,两组ALB、PA水平均较干预前升高,且观察组ALB、PA水平较对照组高(P<0.05)。干预后,观察组母乳喂养率91.30%高于对照组79.17,差异有统计学意义(P<0.05)。观察组并发症总发生率4.35%低于对照组18.75%,差异有统计学意义(P<0.05)。结论:NST用于NICU早产儿中可提高母乳喂养率,改善早产儿营养指标,减少营养相关并发症发生。Objective:To investigate the effect of nutrition support group(NST)on feeding and nutritional status of preterm infants in neonatal intensive care unit(NICU).Methods:A prospective randomized trial was conducted to select 48 premature infants from NICU of Shaoguan first people’s Hospital from March 2018 to may 2019 who were independently checked by newborn pediatricians as the control group,and 46 premature infants from NICU of Shaoguan first people’s Hospital from June 2019 to may 2020 who were checked by NST as the observation group.The feeding status,nutritional status[albumin(ALB),prealbumin(PA)]and the incidence of complications were recorded.Results:There was no significant difference in the levels of ALB and PA between the two groups before intervention(P>0.05);After 2 weeks of intervention,the levels of ALB and PA in the two groups were higher than those before intervention,and the levels of ALB and PA in the observation group were higher than those in the control group(P<0.05).After the intervention,the breastfeeding rate in the observation group was 91.30%higher than 79.17 in the control group,and the difference was statistically significant(P<0.05).The total incidence of complications in the observation group was 4.35%,which was lower than 18.75%in the control group(P<0.05).Conclusion:NST used in NICU preterm infants can improve the breastfeeding rate,improve the nutritional indexes of preterm infants and reduce the incidence of nutrition related complications.
