两种麻醉方法在阴式子宫全切除术中的应用效果比较分析

摘　　要：目的比较腰-硬联合麻醉与硬膜外麻醉在阴式子宫全切除术中的应用效果。方法选择行阴式子宫全切除术的44例患者,依据麻醉方式的不同分为联合麻醉组和一般麻醉组,每组22例。一般麻醉组实施硬膜外麻醉,联合麻醉组实施腰-硬联合麻醉。比较两组镇痛效果、血压水平、不良反应发生率。结果联合麻醉组术中镇痛效果优于一般麻醉组,差异有统计学意义(P<0.05)。麻醉前,两组血压指标比较,差异无统计学意义(P>0.05);麻醉后,联合麻醉组收缩压、舒张压均高于一般麻醉组,差异均有统计学意义(P<0.05)。联合麻醉组不良反应发生率低于一般麻醉组,但是差异无统计学意义(P>0.05)。结论腰-硬联合麻醉在阴式子宫全切除术中效果较好,可以进一步提升镇痛效果,对患者的血压水平影响较小,安全性高,值得在临床中推广与应用。Objective To compare the effects of combined lumbo-epidural anesthesia and epidural anesthesia in total vaginal hysterectomy.Methods A total of 44 patients who underwent total vaginal hysterectomy were divided into combined anesthesia group and general anesthesia group according to different anesthesia methods,22 cases in each group.Epidural anesthesia was performed in the general anesthesia group,and lumbo-epidural anesthesia was performed in the combined anesthesia group.The analgesic effect,blood pressure level and incidence of adverse reactions were compared between the two groups.Results The analgesic effect of combined anesthesia group was better than that of general anesthesia group,the difference was statistically significant(P<0.05).Before anesthesia,there was no significant difference in blood pressure index between the two groups(P>0.05).After anesthesia,the systolic blood pressure and diastolic blood pressure in the combined anesthesia group were higher than those in the general anesthesia group,and the differences were statistically significant(P<0.05).The incidence of adverse reactions in combined anesthesia group was lower than that in general anesthesia group,but the difference was not statistically significant(P>0.05).Conclusion Combined lumbo-epidural anesthesia has a good effect in total vaginal hysterectomy,which can further improve the analgesic effect,and has little influence on the blood pressure level of patients with high safety.It is worthy of promotion and application in clinical practice.
