摘 要:目的探讨不同分级慢性阻塞性肺疾病(COPD)患者调节性T细胞核转录因子叉头状转录因子3(Foxp3)的表达水平及与炎性因子相关性。方法选取2015年1月至2017年1月来院就诊的120例COPD患者纳入观察组,按照病情严重程度分为COPD1~2级组53例、COPD3~4级组67例,并选择同期进行健康体检的健康志愿者50例为对照组。对3组受试者的Foxp3水平、炎性因子[C-反应蛋白(CRP)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、B型脑尿钠肽(BNP)]水平进行检测对比,并采用Pearson检验对COPD患者Foxp3水平与炎性因子水平间相关性进行分析探讨。结果观察组患者的Foxp3水平低于对照组,炎性因子CRP、IL-6、TNF-α、血浆BNP水平均高于对照组,组间比较差异有统计学意义(P<0.05)。观察组内COPD3-4级组患者Foxp3水平低于COPD1-2级组,而CRP、IL-6、TNF-α、BNP水平均高于COPD1-2级组,组间比较差异有统计学意义(P<0.05)。经Pearson相关性检验,Foxp3水平与CRP、IL-6、TNF-α、BNP呈负相关性(r值分别为-0.571、-0.534、-0.593、-0.516,P<0.05)。结论在COPD患者体内调节性T细胞核转录因子Foxp3的表达水平有降低趋势,并随着患者病情分级加重而进一步降低,且Foxp3与炎性因子水平呈负相关性,通过对COPD患者Foxp3、炎性因子水平监测,对于疾病的临床治疗评估有一定的指导作用。Objective To investigate the effects of To investigate the expression of regulatory T cell nuclear transcription factor fork-like transcription factor 3(Foxp3)in patients with chronic obstructive pulmonary disease(COPD)with different grades and its correlation with inflammatory factors.Methods A total of 120 patients with COPD who were treated in our hospital from January 2015 to January 2017 were enrolled as observation group.According to the severity of the disease,the patients were divided into COPD grade 1~2 group(n=53)and COPD grade 3-4 group(n=67),at the same time,the other 50 healthy volunteers who underwent physical examination in our hospital were enrolled as control group.The levels of Foxp3 and inflammatory factors were measured and compared among groups.Pearson test was performed to analyze the correlation between Foxp3 and inflammatory factors.Results The Foxp3 levels in observation group were significantly lower than those in control group,however,the levels of inflammatory factors including C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-alpha(TNF-alpha),plasma B-type sodium encephaluria(BNP)were significantly higher than those in control group(P<0.05).The level of Foxp3 in COPD grade 3~4 group were significantly lower than those in COPD grade 1~2 group,while the levels of CRP,IL-6,TNF-alpha and BNP in COPD grade 1~2 group were significantly higher than those in COPD grade 1~2 group(P<0.05).Pearson correlation test showed that Foxp3 levels were negatively correlated with those of CRP,IL-6,TNF-a and BNP(P<0.05).Conclusion The expression levels of regulatory T cell nuclear transcription factor Foxp3 in patients with COPD are decreased,which are further decreased with the aggravation of disease condition.Moreover Foxp3 levels are negatively correlated with those of inflammatory factors.Monitoring the levels of Foxp3 and inflammatory factors may play a certain guiding role in the clinical evaluation of disease.
