摘 要:目的分析不同气道给药方式联合支气管镜下重组牛碱性成纤维细胞生长因子(rb-bFGF)治疗气管黏膜损伤的效果。方法选取2018年8月至2020年8月收治的173例气管黏膜损伤患者作为研究对象,按照气道给药方式分为气管滴入组84例、雾化吸入组89例。2组患者均行支气管镜下rb-bFGF治疗,气管滴入组在rb-bFGF治疗的基础上采用肾上腺素气管滴入治疗,雾化吸入组患者在rb-bFGF治疗的基础上采用肾上腺素雾化吸入治疗。对比分析2组气管狭窄程度、疼痛程度、肺功能[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、最大肺活量(Vcmax)]、气管黏膜纤维结构,并评价临床疗效。结果与治疗前比较,2组治疗后气管狭窄程度、疼痛评分降低,且雾化吸入组降低程度优于气管滴入组(P<0.05)。与治疗前比较,2组治疗后FEV1、FVC、Vcmax升高,且雾化吸入组升高/降低程度优于气管滴入组(P<0.05)。与气管滴入组比较,雾化吸入组气管黏膜纤维结构异常率较低(P<0.05)。与气管滴入组比较,雾化吸入组治疗显效率较高(P<0.05)。结论与气管滴入给药相比,雾化吸入联合支气管镜下rb-bFGF可有效改善气管黏膜损伤患者气管狭窄程度,减轻疼痛,改善肺功能,效果显著。Objective To investigate the clinical efficacy of different airway medication modes combined with bronchoscopic recombinant bovine basic fibroblast growth factor(rb-bFGF)on tracheal mucosal injury.Methods A total of 173 patients with tracheal mucosal injury who were treated in our hospital from August 2018 to August 2020 were enrolled in the study.According to different airway medication modes,who were divided into tracheal instillation group(n=84)and aerosol inhalation group(n=89).The patients in both groups were treated by RB bFGF under bronchoscope.On that basis,the patients in tracheal instillation group were treated by epinephrine tracheal instillation,however,those in atomization inhalation group were treated by epinephrine atomization inhalation.The tracheal stenosis degree,pain degree,pulmonary function,tracheal mucosal fiber structure as well as clinical efficacy were observed and compared between the two groups.Results Compared with those before treatment,the tracheal stenosis degree and pain scores in both groups were decreased after treatment,moreover,which in atomization inhalation group were superior to those in tracheal instillation group(P<0.05).After treatment,the levels of FEV1,FVC,Vcmax in both groups were significantly increased,moreover,the increase degree in atomization inhalation group was more significant than that in tracheal instillation group(P<0.05).Moreover the abnormal rate of tracheal mucosal fiber structure in atomization inhalation group was significantly lower than that in tracheal instillation group(P<0.05),and the treatment excellent rate in atomization inhalation group was significantly higher than that in tracheal instillation group(P<0.05).Conclusion Compared with intratracheal instillation medication,the aerosol inhalation combined with RB bFGF under bronchoscope can effectively improve the tracheal stenosis degree,relieve pain degree and improve lung function in patients with tracheal mucosal injury.
