二甲双胍联合胰岛素治疗妊娠期糖尿病对血糖、血清胱抑素C、血清同型半胱氨酸水平及妊娠结局的影响

摘　　要：目的分析二甲双胍联合胰岛素治疗妊娠期糖尿病的效果。方法选取本院收治的40例妊娠期糖尿病患者为研究对象,采用随机数字表法将其分为对照组及研究组。对照组20例采用胰岛素治疗,研究组20例在此基础上采用二甲双胍治疗,比较两组临床疗效。结果治疗前,两组血清胱抑素C(CysC)、血清同型半胱氨酸(HCY)水平比较,差异无统计学意义(P>0.05);治疗后,两组CysC、HCY水平均明显低于治疗前,且研究组各指标水平均明显低于对照组,差异有统计学意义(P<0.05)。治疗前,两组空腹血糖(FPG)、糖化血红蛋白(HbA1c)和餐后2 b血糖(2 h PG)水平比较,差异无统计学意义(P>0.05);治疗后,两组FPG、HbA1c、2 h PG水平均明显低于治疗前,且研究组各指标水平均明显低于对照组,差异有统计学意义(P<0.05)。研究组孕产妇并发症发生率、新生儿并发症发生率分别为5.00%、5.00%,均明显低于对照组的35.00%、40.00%,差异有统计学意义(P<0.05)。结论妊娠期糖尿病患者接受二甲双胍联合胰岛素治疗,能有效控制血糖,降低CysC和HCY水平,改善妊娠结局,具有推广价值。Objective To analyze the effect of metformin combined with insulin in the treatment of gestational diabetes mellitus.Methods A total of 40 patients with gestational diabetes mellitus admitted to our hospital were selected as subjects and divided into a control group and a study group by random number table method.20 patients in the control group were treated with insulin,and 20 patients in the study group were treated with metformin on this basis.The clinical efficacy of the two groups was compared.Results Before treatment,there were no significant differences in scrum cystatin C(CysC)and serum homocysteine(HCY)levels between the two groups(P>0.05);after treatment,the levels of CysC and HCY in both groups were significantly lower than before treatment,and the levels of indicators in the study group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of fasting blood glucose(FPG),glycosylated hemoglobin(HbAlc)and 2 h postprandial blood glucose(2 h PG)between the two groups(P>0.05);after treatment,the levels of FPG,HbAlc and 2 h PG in both groups were significantly lower than before treatment,and the levels of all indicators in the study group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).The incidence of maternal complications and neonatal complications in the study group were 5.00%and 5.00%,respectively,which were significantly lower than 35.00%and 40.00%in the control group,the differences were statistically significant(P<0.05).Conclusion Metformin combined with insulin therapy can effectively control blood glucose,reduce the levels of CysC and HCY,and improve the pregnancy outcome in patients with gestational diabetes,which is of popularization value.
