荆防方加减联合针刺治疗风热型荨麻疹疗效观察

摘　　要：目的观察荆防方加减联合针刺治疗风热型荨麻疹的疗效。方法将100例风热型荨麻疹患者按照随机数字表法分为对照组和治疗组各50例。对照组予氯雷他定片治疗,治疗组在对照组治疗基础上加荆防方加减联合针刺治疗,2组均治疗4周。比较2组治疗前后7日荨麻疹活动度评分(UAS7)、中医证候评分、炎症因子[肿瘤坏死因子α(TNF-α)、白细胞介素2(IL-2)、IL-13、白三烯C4(LTC4)]水平变化及临床疗效、随访6个月的复发率。结果治疗后,2组UAS7评分、中医证候各项评分及总分,以及炎症因子TNF-α、IL-2、IL-13、LTC4水平均较本组治疗前明显下降(P<0.05),且治疗组均显著低于对照组(P<0.05)。治疗组总有效率96.00%(48/50),对照组总有效率82.00%(41/50),治疗组总有效率显著高于对照组(P<0.05)。随访6个月,治疗组复发率为8.33%(4/48),对照组复发率为29.27%(12/41),治疗组复发率低于对照组(P<0.05)。结论荆防方加减联合针刺治疗风热型荨麻疹,可显著改善临床症状,抑制炎性反应,提高临床疗效,降低复发率。Objective To observe the curative effect of modified Jingfang Formula combined with acupuncture on urticaria(wind-heat type).Methods Totally 100 patients with urticaria(wind-heat type)were randomly assigned into treatment group(n=50)and control group(n=50).All patients were managed by loratadine tablets,and those in treatment group were additionally treated with modified Jingfang Formula combined with acupuncture.A 4-week treatment was performed to compare Urticaria Activity Score over 7 days(UAS7),traditional Chinese medicine(TCM)syndrome score,inflammatory factor(tumor necrosis factor-α[TNF-α],interleukin 2[IL-2],IL-13,leukotriene C4[LTC4]);the curative effect and 6-month follow-up of recurrence rate were assessed.Results After treatment,UAS7,TCM symptom scores,TNF-α,IL-2,IL-13,LTC4 in groups were notably decreased(P<0.05),treatment group was more prevalent(P<0.05).The overall effective rate in treatment group was better than that in control group(96.00%[48/50]vs 82.00%[41/50],[P<0.05],respectively).The 6-month follow-up of recurrence rate was 8.33%(4/48)in treatment group,lower than that of 29.27%(12/41)in control group(P<0.05).Conclusion For patients with urticaria(wind-heat type),modified Jingfang Formula combined with acupuncture can significantly improve clinical symptoms,inhibit inflammatory response,improve clinical efficacy and reduce recurrence rate.
