化瘀止痛补肾方联合腹腔镜手术治疗肾虚血瘀型子宫内膜异位症的疗效观察

摘　　要：目的观察化瘀止痛补肾方联合腹腔镜手术治疗肾虚血瘀型子宫内膜异位症的临床疗效。方法将100例肾虚血瘀型子宫内膜异位症患者按照随机数字表法分为2组,对照组50例予腹腔镜手术治疗,治疗组50例在对照组的基础上联合化瘀止痛补肾方治疗。2组均治疗3个月经周期,比较2组治疗前后中医证候(包括经期下腹疼痛、小腹冷痛得温痛减、非经期疼痛、月经少经期后延夹血块、形寒肢冷、带下量多色白、大便稀、舌黯红苔薄白或腻及脉弦或沉紧)积分、性激素[包括黄体生成激素(LH)、卵泡刺激素(FSH)及雌二醇(E_(2))]水平变化情况。结果治疗组总有效率96.00%(48/50),对照组总有效率82.00%(41/50),治疗组疗效优于对照组(P<0.05)。与本组治疗前比较,2组治疗后中医证候经期下腹疼痛、小腹冷痛得温痛减、非经期疼痛、月经少经期后延夹血块、形寒肢冷、带下量多色白、大便稀、舌黯红苔薄白或腻、脉弦或沉紧积分及总分均降低(P<0.05),且治疗组治疗后中医证候各项积分及总分均低于对照组(P<0.05)。与本组治疗前比较,2组治疗后LH水平均升高(P<0.05),FSH及E_(2)水平均降低(P<0.05),且治疗组治疗后LH水平高于对照组(P<0.05),FSH及E_(2)水平低于对照组(P<0.05)。结论化瘀止痛补肾方联合腹腔镜手术治疗肾虚血瘀型子宫内膜异位症疗效确切,可有效改善患者的中医证候表现,调节性激素水平,提高临床疗效。Objective To observe the clinical curative effect of Huayu Zhitong Bushen Formula combined with laparoscopic surgery(LS)for endometriosis(EMS)of kidney blood type.Methods A total of 100 EMS patients(kidney blood type)were randomly assigned into the treatment group(n=50)and the control group(n=50).All patients were managed by LS,and those in the treatment group were additionally treated with Huayu Zhitong Bushen Formula.The treatment continued for 3 menstrual cycles,the aim was to compare traditional Chinese medicine(TCM)symptom(lower abdominal pain during menstruation,cold pain of lower abdomen reduced by warm,non-menstrual pain,less menstruation and delayed menstruation with blood clots,cold body and limbs,leukorrhagia large quantity and white color,thin stool,tongue dark red moss thin white or greasy,deep and tight pulse)scores,sex hormone(SH)(luteinizing hormone[LH],follicle-stimulating hormone[FSH],estradiol[E_(2)]).Results The overall effective rate in the treatment group was better than that in the control group(96.00%[48/50]vs 82.00%[41/50],[P<0.05],respectively).After treatment,TCM symptom scores in the both groups were decreased(P<0.05),which were decreased in the treatment group compared with the control group(P<0.05);LH in groups was increased(P<0.05),the treatment group was more common(P<0.05);FSH and E2 in the both groups were decreased(P<0.05),which in the treatment group was more prevalent relative to the control group(P<0.05).Conclusion For EMS patients(kidney blood type),Huayu Zhitong Bushen Formula combined with LS has definite curative effect,can effectively improve TCM syndrome,regulate SH,and improve clinical curative effect.
