针刺联合灌耳疗法治疗原发性痛经临床疗效观察

摘　　要：目的观察针刺联合灌耳疗法治疗原发性痛经的临床疗效。方法将85例原发性痛经患者随机分为2组。治疗组43例予针刺联合灌耳疗法治疗,对照组42例予《金匮要略》温经汤治疗,2组均每个月经周期行经第1 d开始治疗,连续治疗5 d、3个月经周期。比较2组治疗前及治疗1、2、3个疗程和随访时疼痛视觉模拟评分(VAS)、COX痛经症状评分量表(CMSS)评分,并比较2组第1个疗程止痛起效时间和临床疗效。结果治疗1、2、3个疗程和随访时,2组疼痛VAS、CMSS疼痛程度评分及持续时间评分均较本组治疗前降低(P<0.05),且治疗组疼痛VAS均低于对照组同期(P<0.05),治疗1、2、3个疗程治疗组CMSS疼痛程度评分及持续时间评分均低于对照组同期(P<0.05)。第1个疗程,治疗组止痛起效时间短于对照组(P<0.01)。治疗组临床疗效优于对照组(P<0.01)。结论针刺联合灌耳疗法治疗原发性痛经的效果较服用温经汤更好,且无不良反应,患者易接受。Objective To observe the clinical efficacy of acupuncture combined with ear irrigation for primary dysmenorrhea(PD).Methods PD patients(n=85)were assigned to treatment group(n=43)and traditional Chinese medicine(TCM)group(n=42),in which they were treated with acupuncture combined with ear irrigation and Wenjing Decoction in Synopsis of the Golden Chamber,respectively.The treatment continued for 3 menstrual cycles(treatment started on the first day of each menstrual cycle and continued for 5 days).The aim was to compare the visual analogue scale(VAS)for pain and the Cox Menstrual Symptom Scale(CMSS)before treatment and after 1,2 and 3 courses of treatment and during follow-up,initial time of relieve pain after 1 course of treatment;the curative effect was assessed.Results After 1,2 and 3 courses of treatment and during follow-up,VAS,CMSS(severity of pain and duration time)scores in groups were decreased(P<0.05),VAS in treatment group was decreased than that in concurrent control group(P<0.05);CMSS(severity of pain and duration time)scores after 1,2 and 3 courses of treatment in treatment group were decreased compared with concurrent control group(P<0.05).Compared with control group,the initial time of relieve pain after 1 course of treatment in treatment group was shorter(P<0.01),the curative effect in treatment group was better(P<0.01).Conclusion Acupuncture combined with ear irrigation is more effective for PD patients than Wenjing Decoction,and with no adverse reactions,it is easy for patients to accept.
