经阴道子宫输卵管实时三维超声造影诊断输卵管源性不孕的价值研究

摘　　要：目的:探讨经阴道子宫输卵管实时三维超声造影诊断输卵管源性不孕的价值。方法:选取2020年2月-2022年2月徐州九龙妇产医院收治的92例疑似输卵管源性不孕患者作为研究对象,均接受经阴道子宫输卵管实时三维超声造影检查,以腹腔镜下美蓝通染液试验结果为金标准,评价三维超声造影诊断输卵管通畅性的符合率及诊断输卵管源性不孕的敏感度、特异度及准确率,观察不良反应发生情况。结果:经阴道子宫输卵管实时三维超声造影诊断输卵管通畅的符合率为99.03%,通而不畅的符合率为64.00%,阻塞的符合率为91.07%,总符合率为91.85%。经阴道子宫输卵管实时三维超声造影诊断输卵管源性不孕的敏感度为98.08%,特异度为96.21%,准确度为96.74%,阳性预测值为91.07%,阴性预测值99.22%。经阴道子宫输卵管实时三维超声造影检查的不良反应发生率低于腹腔镜下美蓝通染液试验,差异有统计学意义(P<0.05)。结论:经阴道子宫输卵管实时三维超声造影检查可准确评估女性输卵管的通畅性,以阻塞为标准评价输卵管源性不孕的敏感度及特异度高,不良反应少,值得临床应用并予以推广。Objective:To explore the value of transvaginal hysterosalpingography real-time three-dimensional contrast-enhanced ultrasound in the diagnosis of tubal infertility.Methods:A total of 92 patients with suspected tubal infertility received by Xuzhou Kowloon Maternity Hospital from February 2020 to February 2022 were selected as the research subjects.They all received transvaginal hysterosalpingography real-time three-dimensional contrast-enhanced ultrasound.The results of laparoscopic methylene blue dye test were used as the gold standard to evaluate the coincidence rate of three-dimensional contrast-enhanced ultrasound in the diagnosis of fallopian tube patency and the sensitivity,specificity and accuracy of diagnosing tubal infertility.The occurrence of adverse reactions was observed.Results:The coincidence rate of transvaginal hysterosalpingography real-time three-dimensional contrast-enhanced ultrasound in the diagnosis of fallopian tube patency was 99.03%,the coincidence rate in the diagnosis of incomplete patency was 64.00%,the coincidence rate in the diagnosis of obstruction was 91.07%,and the total coincidence rate was 91.85%.The sensitivity of transvaginal hysterosalpingography real-time three-dimensional contrastenhanced ultrasound in the diagnosis of tubal infertility was 98.08%,the specificity was 96.21%,the accuracy was 96.74%,the positive predictive value was 91.07%,and the negative predictive value was 99.22%.The incidence of adverse reactions of transvaginal hysterosalpingography real-time three-dimensional contrast-enhanced ultrasound was lower than that of laparoscopic methylene blue dye test,and the difference was statistically significant(P<0.05).Conclusion:Transvaginal hysterosalpingography real-time three-dimensional contrast-enhanced ultrasound can accurately evaluate the patency of fallopian tubes in women.Taking the obstruction as the criteria,the sensitivity and specificity for evaluating tubal infertility are high,and there are few adverse reactions.It is worthy of clinical application and
