基于异病同治探讨肝胃百合汤治疗肝胃不和型反流性咽喉炎

摘　　要：反流性咽喉炎是由于胃内容物反流到咽部,刺激损伤咽部黏膜,以致咽部不适,或有物梗之,或咽干咽痒,或干咳声嘶,或频繁清嗓的疾病。中医称之为“慢喉痹”“梅核气”“吞酸”等,有学者研究发现肝胃不和型反流性咽喉炎在临床最为常见,其病机多责之肝气郁结,中焦气机不畅,胃气上逆以泛咽喉。基于中医异病同治,证同治亦同的原则,在治疗肝胃不和型反流性咽喉炎时,抓住关键病机,从肝胃论治。治法疏肝理气,和胃利咽,方用名老中医夏度衡经验方肝胃百合汤加减。附以病例详细剖析,为反流性咽喉炎的临床中医治疗提供新思路。Reflux pharyngitis is caused by reflux of gastric contents into the pharynx,stimulation and injury of pharyngeal mucosa,resulting in pharyngeal discomfort,sense of obstruction,dry pharynx itching,dry cough hoarse,frequent throat clearing disease.Traditional Chinese medicine called“chronic throat obstruction”,“globus hystericus”,“acid regurgitation”.Scholars have found that hepato-gastric disharmony reflux pharyngitis is the most common pattern in clinical,which caused by stagnation of liver qi that blocking qi function of the middle burner makes the adverse rising of stomach qi injure the throat.Based on the traditional Chinese medicine principle“treating different diseases with the same method”and“the same treatment for the same syndrome”,in the treatment of the liver and stomach for the liver-stomach disharmony type reflux pharyngitis,seized the key pathogenesis,treat from liver and stomach.The treatment method is smoothing the liver and regulating qi,harmonizing stomach and comforting pharynx,and used Ganwei Baihe decoction,which is an experience prescription from prestigious doctor of traditional Chinese medicine Xia Duheng.The detailed cases provides a new idea for the clinical treatment of reflux pharyngitis of traditional Chinese medicine.
