身痛逐瘀汤联合针刺对膝关节置换术后患者疼痛的影响

摘　　要：目的身痛逐瘀汤联合健侧针刺对膝关节置换术后患者疼痛的影响。方法将中国医科大学附属第一医院收治的114例膝关节置换术后的患者作为研究对象,随机分为3组:对照组、患侧针刺组和健侧针刺组,每组38例,均服用身痛逐瘀汤,选用足三里、阴陵泉、阳陵泉,使用2 Hz的频率电刺激治疗。结果3组患者治疗前相关评分术前比较,患者在视觉模拟评分(VAS)、健康调查简表(SF-36)、膝活动度(ROM)、美国纽约特种外科医院(HSS)功能评分方面,差异无统计学意义(P>0.05);治疗14 d后,与对照组相比,患侧针刺组和健侧针刺组在VAS评分上降低(P<0.05);与对照组相比,患侧针刺组和健侧针刺组在SF-36评分、ROM度数、HSS评分上均提高(P<0.05)。结论健侧针刺联合身痛逐瘀汤对膝关节置换术后患者的疼痛具有改善作用。Objective To observe the effect of Shentong Zhuyu decoction combined with acupuncture on the contralateral side on pain after knee arthroplasty.Methods 114 patients after knee replacement in the First Hospital of China Medical University were randomly divided into control group,acupuncture group on the affected side and acupuncture group on the uninjured side,with 38 in each group.All patients took Shentong Zhuyu decoction and were treated with 2Hz frequency electrical stimulation at Zusanli(ST36),Yinlingquan(SP9)and Yanglingquan(GB34).Results There was no significant difference in Visual analogue score(VAS),the MOS item short from health survey(SF-36),knee range of motion(ROM),functional score of New York special surgery hospital(HSS)(P>0.05).After 14 days of treatment,the VAS scores of the affected side acupuncture group and the healthy side acupuncture group were lower than those of the control group(P<0.05).Compared with the control group,the SF-36 scores,ROM degree and HSS scores of the acupuncture group on the affected side and the acupuncture group on the healthy side were improved(P<0.05).Conclusion Acupuncture at healthy side combined with Shentong Zhuyu decoction can improve the pain of patients after knee replacement.
