摘 要:目的:探讨西红康对2型糖尿病肾病大鼠肾足细胞功能的影响。方法:选择SPF级8周龄雄性Wistar大鼠50只大鼠随机分为空白对照组、DN模型对照组、西红康大剂量组、西红康中剂量组、西红康小剂量组,连续给药30 d后检测大鼠空腹血糖(FBG)、晚期氧化蛋白产物(AOPPs)与尿微量白蛋白(mALB)水平,各组大鼠在麻醉状态下取肾皮质,检测并比较足细胞计数、足突平均宽度、足突融合率及肾小球基底膜平均厚度。结果:干预30 d后,各剂量西红康治疗组与模型对照组比较,FBG、mALB与AOPPs水平均明显降低,差异具有统计学意义(P<0.05),但各剂量治疗组间比较差异无统计学意义(P>0.05)。各剂量西红康治疗组与模型对照组比较,足细胞计数、足突宽度、足突融合率与肾小球基底膜平均厚度(GBM)均明显增高或降低,差异具有统计学意义(P<0.05),西红康大、中剂量治疗组与小剂量治疗组比较,足细胞计数、足突宽度、足突融合率与GBM比较差异均具有统计学意义(P<0.05)。随着西红康剂量的增加,大鼠肾小球GBM增厚、足突断裂、排列不整齐、足突融合等现象明显减轻。结论:西红康可降低2型糖尿病肾病大鼠的FBG、AOPPs与mALB水平,还可有效改变其肾足细胞超微结构,达到保护肾脏的作用。Objective:To investigate the effect of Xihongkang on renal podocyte function in rats with type 2 diabetic nephropathy.Methods:Fifty SPF 8-week-old male Wistar rats were randomly divided into blank control group,DN model control group,Xihongkang small,medium and high dose groups.After continuous administration for 30 days,the levels of FBG,AOPPs and mALB in the rats were detected.The renal cortex was taken from the rats in each group under anesthesia,and the podocyte count,the average width of the foot process,the fusion rate of the foot process and the average thickness of the glomerular basement membrane were detected and compared.Results:After 30 days of intervention,compared with the model control group,the levels of FBG,mALB and AOPPs in each dose of Xihongkang treatment group were significantly decreased,and the difference was statistically significant(P<0.05),but there were differences between the treatment groups at each dose no statistical significance(P>0.05).Compared with the model control group,the podocyte count,foot process width,foot process fusion rate and GBM in each dose of Xihongkang treatment group were significantly increased or decreased,and the difference was statistically significant(P<0.05).Compared with the low-dose treatment group,the podocyte count,foot process width,foot process fusion rate and GBM were significantly different in the dose treatment group(P<0.05).With the increase of the dose of Xihongkang,the rat glomerular GBM thickened,the foot process was broken,the arrangement was irregular,and the foot process fusion and other phenomena were obviously alleviated.Conclusion:Xihongkang can reduce the levels of FBG,AOPPs and mALB in rats with type 2 diabetic nephropathy,and can also effectively protect the kidneys from the ultrastructure of renal podocytes.
