加减柴胡温经汤治疗围绝经期肝郁阴虚夹瘀型失眠疗效及对患者身心症状、抑郁情绪和血清性激素水平的影响

摘　　要：目的:探究加减柴胡温经汤治疗围绝经期肝郁阴虚夹瘀型失眠疗效,并重点分析其对患者身心症状、抑郁情绪和血清性激素水平的影响。方法:选取围绝经期肝郁阴虚夹瘀型失眠患者共计104例,采用随机数字表法将上述患者分为观察组(n=52)以及对照组(n=52),对照组患者服用坤泰胶囊治疗;观察组患者给予加减柴胡温经汤治疗;统计两组的治疗总有效率、治疗前后匹兹堡睡眠质量指数(PSQI)、抑郁自评量表(SDS)评分、焦虑自评量表(SAS)评分、雌二醇(E_(2))、卵泡刺激素(FSH)、黄体生成素(LH)、白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)以及肿瘤坏死因子(TNF-α)水平并进行比较;记录用药不良反应。结果:观察组治疗总有效率为96.15%,显著高于对照组的78.85%,差异有统计学意义(P<0.05);治疗后,两组PSQI、SDS及SAS评分比较治疗前均出现降低,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组患者的E_(2)水平升高,FSH、LH水平降低,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组血清IL-6、IL-1β及TNF-α均降低,观察组低于对照组(P<0.05);两组均未出现显著用药不良反应。结论:加减柴胡温经汤用于治疗围绝经期肝郁阴虚夹瘀型失眠效果良好,能够有效改善身心症状、缓解抑郁情绪并调整血清性激素水平。Objective:To explore the curative effect of modified Chaihu Wenjing decoction on perimenopausal insomnia of liver-stagnation yin deficiency stasis type,and to emphatically analyze its influences on psychosomatic symptoms,depression and serum sex hormones.Methods:A total of 104 patients with perimenopausal insomnia of liver-stagnation yin deficiency stasis type were divided into observation group(n=52)and control group(n=52).The control group was given routine treatment,while observation group was additionally given modified Chaihu Wenjing decoction.The total response rate of treatment,scores of pittsburgh sleep quality index(PSQI),self-rating depression scale(SDS)and self-rating anxiety scale(SAS),levels of estradiol(E_(2)),follicle stimulating hormone(FSH),luteinizing hormone(LH),interleukin-6(IL-6),interleukin-1β(IL-1β)and tumor necrosis factorα(TNF-α)before and after treatment were compared between the two groups.The occurrence of adverse reactions during medication in both groups was statistically analyzed.Results:The total response rate of treatment in observation group was significantly higher than that in control group(96.15%vs 78.85%)(P<0.05).After treatment,scores of PSQI,SDS and SAS in both groups were decreased,which were lower in observation group than control group(P<0.05).After treatment,E_(2) was increased,while FSH and LH levels were decreased in both groups.And changes of the above indexes were more significant in observation group than control group(P<0.05).After treatment,levels of serum IL-6,IL-1βand TNF-αin both groups were decreased,which were lower in observation group than control group(P<0.05).There were no significant adverse drug reactions in either group.Conclusion:Curative effect of modified Chaihu Wenjing decoction is good on perimenopausal insomnia of liver-stagnation yin deficiency stasis type,which can effectively improve psychosomatic symptoms,relieve depression and adjust levels of serum sex hormones.
