基于640层CT下不同型号的静脉留置针在头颈CT血管成像中的应用比较

摘　　要：目的:探讨不同型号静脉留置针在头颈640层CT血管成像(CT angiography,CTA)中的应用效果。方法:选取2021年7月-2022年6月在重庆大学附属江津医院进行头颈CTA检查的患者565例为研究对象。将2021年7月-12月收治的265例患者设为对照组,2022年1月-6月收治的300例患者设为试验组。对照组采用22G静脉留置针,试验组采用18G静脉留置针,完成头颈CTA检查。比较两组图像质量主观和客观指标、静脉穿刺成功率、对比剂外渗率、注射部位疼痛感。结果:试验组管腔边缘锐利度、血管与周围结构的对比度、图像清晰度评分均显著高于对照组(P<0.01)。两组上腔静脉、颈内静脉、大脑大静脉、主动脉弓、颈总动脉、颈内动脉、大脑中动脉的CT值比较,差异具有统计学意义(P<0.05)。试验组注射部位疼痛感评分显著低于对照组(P<0.01)。两组静脉穿刺成功率和对比剂外渗率比较,差异无统计学意义(P>0.05)。结论:基于640层CT下,行头颈部CTA检查中采用18G静脉留置针完成静脉碘对比剂团注,能更好地与检查时间和注射速度相匹配,可有效提升图像质量,减少对比剂残留,提高检查的成功率,同时提升患者的舒适感。Objective To investigate the application effect of different types of intravenous indwelling needles in 640 slice computed tomography angiography(CTA)of head and neck.Methods 565 patients who underwent head and neck CTA in Jiangjin Hospital affiliated to Chongqing University from July 2021 to June 2022 were selected as the study subjects.265 patients admitted from July to December 2021 were set as the control group,and 300 patients admitted from January to June 2022 were set as the experimental group.22G intravenous indwelling needle was used in the control group,and 18G intravenous indwelling needle was used in the experimental group,completed head and neck CTA.The subjective and objective indexes of image quality,the success rate of venipuncture,the extravasation rate of contrast agent,and the pain sensation at the injection site were compared between the two groups.Results The experimental group had significantly higher lumen edge sharpness,contrast between vessels and surrounding structures,and image definition scores than the control group(P<0.01).The CT values of superior vena cava,internal jugular vein,great cerebral vein,aortic arch,common carotid artery,internal carotid artery,and middle cerebral artery were compared between the two groups,and the difference was statistically significant(P<0.05).The pain score of the injection site in the experimental group was significantly lower than that in the control group(P<0.01).There was no significant difference in the success rate of venipuncture and the extravasation rate of contrast agent between the two groups(P>0.05).Conclusion Based on 640 slice CT,18G intravenous indwelling needle was used to complete the intravenous iodine contrast agent injection in the head and neck CTA examination,which can better match the inspection time and injection speed,effectively improve the image quality,reduce the residual contrast agent,improve the success rate of the examination,and improve the comfort of patients.
