摘 要:目的探讨UF-5000尿流式分析仪在尿路感染患者筛查中的应用价值。方法收集2020年10月—2021年10月本院住院及门诊疑似尿路感染患者尿液标本565例,分别进行尿培养和尿常规检测,记录亚硝酸盐(NIT)和白细胞酯酶(LEU)、尿白细胞计数(U-WBC)、尿细菌计数(U-BACT)结果及细菌报警信息(BACT-Info),对U-WBC、U-BACT及四项联合检测结果绘制ROC曲线,评价其对尿路感染的诊断效能;并比较BACT-Info与尿培养结果符合性。结果细菌培养阳性者123例(女性72例,男性51例),其中以大肠埃希菌感染者占比最高;对U-BACT和U-WBC绘制ROC曲线,得出的曲线下面积(AUC)分别为0.906和0.645,两者的诊断阈值、特异度和敏感度分别为U-BACT:84.550个/μl(0.837,0.837),U-WBC:34.750个/μl(0.694,0.537),而四项联合检测时AUC为0.843,且特异度(0.921)明显高于单项检测;在真阴性患者及革兰阴性菌感染者间BACT-Info与尿培养的符合率分别达到了89%和88%。结论NIT和LEU单一检测干扰因素较多,U-BACT比U-WBC对尿路感染有更好的诊断价值,而4项联合检测更能提高其诊断效能;UF-5000的BACT-Info与尿培养符合率较高,特别对早期快速筛查真阴性患者及革兰阴性菌感染者的用药提供一定的指导价值。Objective To investigate the application value of UF-5000 urine flow analyzer in screening patients with urinary tract infection.Methods A total of 565 urine samples from suspected urinary tract infection inpatients and outpatients in our hospital from October 2020 to October 2021 were collected for urine culture and routine urine testing.The results of nitrite(NIT)and leucocyte esterase(LEU),urine white blood cell count(U-WBC),urine bacteria count(U-BACT)and bacterial alarm information(BACT-Info)were recorded.ROC curves were drawn for U-WBC,U-BACT and the combined results of the four tests to evaluate their diagnostic efficacy for urinary tract infection.The conformance between BACT-Info and urine culture was compared.Results A total of 123 positive cases(72 females and 51 males)were prepared,among which Escherichia coli infection accounted for the highest proportion.ROC curves were drawn for U-BACT and U-WBC,and the area under the curve(AUC)was 0.906 and 0.645,respectively.The diagnostic threshold,specificity and sensitivity of U-BACT and U-WBC were respectively 84.550/ul(0.837,0.837)and 34.750/ul(0.694,0.537),while the AUC was0.868 for the four combined tests,and the sensitivity(0.939)was significantly higher than that for the single test.In true negative patients and gram-negative bacteria infection between BACT-Info and the accuracy of urine culture respectively reached89%and 88%.Conclusion Single NIT or LEU detection has more interference factors,U-BACT has better diagnostic value for urinary tract infection than U-WBC,and the four combined test improves the diagnostic efficiency.UF-5000’s BACT-Info has a high coincidence rate with urine culture,which can especially provide guidance for the early and rapid screening of true negative patients and the medicine using for gram-negative bacteria infection.
