摘  要:目的分析腹泻患者类志贺邻单胞菌的临床致病性及其药敏情况,以指导临床合理用药。方法选择2021年5月—10月肠道门诊分离的15株类志贺邻单胞菌为实验菌株。采用MALDI-TOF MS质谱技术进行菌种鉴定及同源性分析;VITKE 2进行药敏分析,部分中介药物使用E-TEST法复查;手工凝集法进行志贺菌4种多价血清凝集。结果类志贺邻单胞菌检出率为1.3%(15/1122),其中1株与多价血清交叉凝集阳性,1株为产H_(2)S突变株,有2株与其他菌混合感染。药敏结果显示对大多数药物敏感,但对喹诺酮类、磺胺类抗生素耐药率高,分别为46.7%、53.3%。VITEK MS RUO数据库同源分析显示菌株主体上为非同源性关系。结论类志贺邻单胞菌是夏季沿海地区腹泻重要病原菌,该菌可与其他腹泻病原菌混合感染,产H_(2)S突变菌株可能毒力增强。目前类志贺邻单胞菌对喹诺酮类抗生素耐药突出,临床需高度重视。Objective This study aims to analyze the clinical pathogenicity and drug sensitivity of Plesiomonas shigelloides from diarrhea patients,and to guide clinical rational drug use.Methods Fifteen strains of Plesiomonas shigelloides isolated from intestinal outpatients during May-October in 2021 were selected as experimental strains.Species identification and homology analysis were performed by MALDI-TOF MS.VITKE 2 was used for drug sensitivity analysis,and some intermediary drugs were reviewed by E-test.Four kinds of Shigella polyvalent serum were agglutinated by manual agglutination.Results The detection rate of Plesiomonas shigelloides was 1.3%(15/1122).One strain was positive cross reaction agglutination for polyvalent serum,one isolate was H_(2)S-producing mutant strain,and two strains were mixed with other bacteria.Drug sensitivity results showed that it was sensitive to most of the drugs,but the drug resistance rate to quinolones(46.7%)and sulfonamides(53.3%)was high.VITEK MS RUO database’s homological analysis showed that the strains were generally non-homological.Conclusion Plesiomonas shigelloides is an important pathogen of diarrhea in coastal areas in summer.The bacteria can cause co-infection with other pathogenic bacteria.The mutant strains producing H_(2)S may be more virulent.The drug resistance of Plesiomonas shigelloides to quinolone antibiotics is prominent,and clinical attention should be paid to it.
