摘  要:目的探究肺结核患者血清中微小RNA-27a(miR-27a)及干扰素-β(IFN-β)表达水平变化及其临床意义。方法选取2019年2月—2021年10月于鄂州市第三医院接受治疗的89例肺结核患者作为肺结核组,并根据随访资料将患者分为疾病稳定组24例和疾病活动组65例;另选取同期于本院健康体检的86例体检者作为对照组。采用实时荧光定量-聚合酶链式反应(RT-qPCR)法检测所有研究对象血清miR-27a表达水平,ELISA法检测外周血单个核细胞中IFN-β水平;肺结核病情程度与miR-27a、IFN-β水平的相关性采用Spearman法分析;血清miR-27a及IFN-β水平检测在肺结核病情诊断中的价值采用受试者工作特征曲线(ROC曲线)分析。结果疾病活动组肺结核患者血清miR-27a及IFN-β水平显著低于疾病稳定组患者及对照组,疾病稳定组肺结核患者血清miR-27a及IFN-β水平显著低于对照组(P<0.05)。肺结核病情程度与miR-27a及IFN-β水平均成负相关(r值分别为-0.660、-0.494,P<0.05)。血清miR-27a及IFN-β诊断肺结核病情的曲线下面积(AUC)分别为0.891(95%CI:0.792~0.990)、0.751(95%CI:0.616~0.885),二者联合诊断肺结核病情的AUC为0.932(95%CI:0.853~0.997)。结论肺结核患者血清中miR-27a及IFN-β表达水平异常降低,且与病情程度相关,血清miR-27a及IFN-β水平检测对肺结核患者病情诊断的临床意义重大。Objective This study aims to explore the level changes of microRNA-27 a(miR-27 a)in the serum and interferon-β(IFN-β)of patients with pulmonary tuberculosis and their clinical significance.Methods Eighty-nine tuberculosis patients who were treated in the Third Hospital of Ezhou City from February 2019 to October 2021 were selected as the tuberculosis group.According to the follow-up data,the patients were divided into stable disease group(n=24)and active disease group(n=65).In addition,86 persons who received physical examination in our hospital during the same period were selected as the control group.The serum miR-27 a level of all subjects was detected by real-time fluorescence quantitative-polymerase chain reaction(RT-qPCR)method,and the IFN-βlevel in peripheral blood mononuclear cells was detected by ELISA;the correlation between the degree of pulmonary tuberculosis and the level of miR-27 a and IFN-βwas analyzed by Spearman’s method;the value of serum miR-27 a and IFN-βlevel detection in the diagnosis of pulmonary tuberculosis was analyzed by receiver operating characteristic curve(ROC curve).Results The levels of serum miR-27 a and IFN-βin active group were significantly lower than those in stable group and control group,and the levels of serum miR-27 a and IFN-βin stable group were significantly lower than those in control group(P<0.05).The degree of pulmonary tuberculosis was negatively correlated with the level of miR-27 a and IFN-β(r=-0.660,r=-0.494,P<0.05).The area under the curve(AUC)of serum miR-27 a and IFN-βin the diagnosis of pulmonary tuberculosis was 0.891(95%CI:0.792-0.990)and 0.751(95%CI:0.616-0.885),respectively,and the two combined diagnosis of tuberculosis was 0.932(95%CI:0.853-0.997).Conclusion The levels of miR-27 a and IFN-βin the serum of pulmonary tuberculosis patients are abnormally decreased,which is related to the severity of the disease.The detection of serum miR-27 a and IFN-βlevel is of great clinical significance in the diagnosis of pulmonary tuberculosis patients.
