外周血中性粒细胞参数对儿童病毒性流感合并细菌感染的临床诊断价值

摘　　要：目的 探讨Sysmex XN-9000^(TM)全自动血液分析仪外周血中性粒细胞参数NEUT-WX和NEUT-WY在对儿童病毒性流感合并细菌感染临床诊断的价值。方法 共收集190例单纯病毒性流感患儿和225例病毒性流感合并细菌感染的患儿,用Sysmex XN9000全自动血液分析仪检测NEUT-WX、NEUT-WY,同期检测超敏C-反应蛋白((hs-CRP)、中性粒细胞碱性磷酸酶,以同期健康体检的158例儿童作为对照,比较3组的NEUT-WX、NEUT-WY、hs-CRP、NAP积分结果。结果 儿童病毒性流感合并细菌感染组NEUT-WX、NEUT-WY、hs-CRP、NAP积分都高于对照组,单纯病毒性流感组的NEUT-WX、NEUT-Y略低于对照组,hs-CRP、NAP略高于对照组,将3组数据进行统计学分析,差异均有统计学意义(P<0.01)。通过分析ROC曲线,得到NEUT-WX、NEUT-WY在检测病毒性流感合并细菌感染中的敏感度分别为0.796、0.796,特异度分别为0.892、0.886,略高于hs-CRP、NAP。结论 NEUT-WX、NEUT-WY对儿童病毒性流感合并细菌性感染的早期诊断有一定的临床价值。Objective To explore the clinical diagnostic value of the neutrophil parameter including NEUT-WX and NEUT-WY by Sysmex XN-9000in children with viral influenza combined with bacterial infection.Methods A total of 190 children with pure viral influenza and 225 children with viral influenza combined with bacterial infection were conducted by the Sysmex XN-9000automatic blood analyzer for the detection of NEUT-WX and NEUT-WY;high sensitive C-reactive protein(hs-CRP)and neutrophil alkaline phosphatase were detected at the same time.In the same period,158 healthy children were used as normal controls,and the results of NEUT-WX,NEUT-WY,hs-CRP,NAP points among the three groups were compared.Results The scores of NEUT-WX,NEUT-WY,hs-CRP and NAP in children with viral influenza combined with bacterial infection were higher than those in the control group.The NEUT-WX and NEUT-Y of the simple viral influenza group were slightly lower than those in the control group.The data of the three groups were statistically analyzed,and the differences were statistically significant(P<0.01).ROC curve analysis showed that the sensitivity and specificity of NEUT-WX and NEUT-WY in the detection of viral influenza combined with bacterial infection were 0.796 and 0.796,respectively,and their specificities were 0.892 and 0.886,which were slightly higher than hs-CRP and NAP.Conclusion NEUT-WX and NEUT-WY have certain clinical value in the early diagnosis of viral influenza complicated with bacterial infection in children.
