活血降浊方对短暂性脑缺血发作伴颈动脉斑块的治疗效应

摘　　要：目的探讨活血降浊方在短暂性脑缺血发作伴有颈部动脉粥样硬化斑块治疗中的临床疗效。方法选取2019年10月—2021年3月收治的60例短暂性脑缺血发作伴颈部动脉粥样硬化斑块患者,依据治疗方案随机将患者分成对照组和试验组,对照组服用阿司匹林联合阿托伐他汀,试验组在对照组基础上加服活血降浊方治疗,两组比较疗效。结果两组治疗前颈动脉斑块面积、颈动脉内-中层厚度及斑块积分比较,差异无统计学意义(P>0.05);试验组治疗后颈动脉斑块面积、颈动脉内-中层厚度及斑块积分小于对照组(P<0.05);两组治疗前血小板聚集率、C-反应蛋白、纤维蛋白原、总胆固醇及低密度脂蛋白比较,差异无统计学意义(P>0.05);试验组治疗后血小板聚集率、C-反应蛋白、纤维蛋白原、总胆固醇及低密度脂蛋白低于对照组(P<0.05);试验组治疗后高密度脂蛋白高于对照组(P<0.05)。结论短暂性脑缺血发作伴颈部动脉粥样硬化斑块患者在阿司匹林联合阿托伐他汀治疗基础上加用活血降浊方治疗效果理想,值得推广。Objective To investigate the clinical effect of Huoxue Jianyzhuo Decoction(活血降浊方)in the treatment of Transient cerebral ischemia with carotid plaques and its influence on hematological indexes.Methods Data of 60 patients with transient cerebral ischemia associated with carotid artery plaques admitted from October 2019 to March 2021 were analyzed,Patients were divided into control group and experimental group according to different treatment regimens,The control group was treated with aspirin combined with atorvastatin,and the experimental group was treated with Huoxue Jianyzhuo Decoction(活血降浊方)on the basis of the control group,and the efficacy of the two groups was compared.Results Before treatment,there were no significant differences in carotid artery plaque area,carotid intima-media thickness and plaque Crouse score between two groups(P>0.05);after treatment,carotid artery plaque area,carotid intima-media thickness and plaque Crouse score in the experimental group were significantly lower than those in the control group(P<0.05);there were no significant differences in platelet aggregation rate,plasma fibrinogen,C-reactive protein,total cholesterol and low density lipoprotein between the two groups(P>0.05);after treatment,the platelet aggregation rate,plasma fibrinogen,C-reactive protein,total cholesterol and low density lipoprotein in experimental group were significantly lower than those in control group(P<0.05);The high density lipoprotein in the experimental group was significantly higher than that in the control group(P<0.05).Conclusion On the basis of aspirin combined with atorvastatin in the treatment of patients with transient cerebral ischemia associated with carotid artery plaque,the combined application of Huoxue Jianyzhuo Decoction(活血降浊方)has an ideal therapeutic effect and is worthy of clinical promotion.
