超微针刀联合加味羌活汤对寒湿痹阻型颞下颌关节紊乱病的临床运用分析

摘　　要：目的 探讨超微针刀联合加味羌活汤对寒湿痹阻型颞下颌关节紊乱病的影响。方法 纳入96例寒湿痹阻型颞下颌关节紊乱病患者,随机分为两组,每组48例,对照组采用超微针刀治疗,治疗组在对照组基础上加用加味羌活汤治疗,两组均治疗2周,观察两组患者治疗后临床疗效、疼痛视觉模拟评分(visual analogue scale, VAS)、最大张口度(maximum mouth opening, MMO)、颞下颌关节功能障碍指数(dysfunction index, DI)、肌肉压痛指数(palpation index, PI)、颞下颌关节紊乱指数(craniomandibular index, CMI)及不良反应发生情况。结果 治疗后,治疗组总有效率为91.67%(44/48),显著高于对照组75.00%(36/48),差异具有统计学意义(P<0.05);治疗后,两组VAS评分均低于治疗前(P<0.05),且治疗组低于对照组(P<0.05);两组MMO均较治疗前明显改善(P<0.05),且治疗组优于对照组(P<0.05);两组DI、PI及CMI评分均低于治疗前(P<0.05),且治疗组低于对照组(P<0.05);两组均未发生严重不良反应,差异无统计学意义(P>0.05)。结论 超微针刀联合加味羌活汤治疗寒湿痹阻型颞下颌关节紊乱病效果显著,可有效改善患者临床症状及安全有效。Objective To investigate the effect of ultra-micro needle knife combined with Modified Qianghuo Decoction(加味羌活汤)on temporomandibular joint disorder of cold-dampness blocking type.Methods Ninety-six patients with cold-dampness obstruction type of temporomandibular joint disorder were randomly divided into two groups,48 cases in each group.The control group was treated with ultra-micro needle knife,and the treatment group was treated with Modified Qianghuo Decoction on the basis of the control group.Both groups were treated for 2 weeks.The clinical efficacy,visual analogue scale(VAS),maximum mouth opening(MMO)and dysfunction index(DI),palpation index(PI),craniomandibular index(CMI)and adverse reactions were compared.Results After treatment,the total effective rate of the treatment group was 91.67%(44/48),which was significantly higher than 75.00%(36/48)of the control group.The difference was statistically significant(P<0.05).After treatment,the VAS scores of the two groups were lower than those before treatment(P<0.05).And the scores of the treatment group was lower than that of the control group(P<0.05).The MMO value of the two groups was significantly improved compared with that before treatment(P<0.05),and that of the treatment group was better than that of the control group(P<0.05).The DI,PI and CMI scores of the two groups were lower than those before treatment(P<0.05),and those of the treatment group were lower than those of the control group(P<0.05).There were no serious adverse reactions in the two groups,and the difference was not statistically significant(P>0.05).Conclusion The combination of ultra-micro needle knife combined with Modified Qianghuo Decoction is effective in treating temporomandibular joint disorder of cold-dampness blocking type,which can effectively improve the clinical symptoms of patients and it is safe and effective.
