尹莲芳教授应用健脾补肺汤治疗稳定期慢性阻塞性肺疾病脾肺两虚证临床观察

摘　　要：目的 研究尹莲芳教授应用健脾补肺汤内服配合伏九贴治疗稳定期慢性阻塞性肺疾病(chronic obstructive pulmonary disease, COPD)脾肺两虚证患者的临床疗效。方法 选取2020年1月-2020年9月医院收治的稳定期COPD脾肺两虚证患者140例为研究对象,随机数字表法将患者分为两组分别70例,对照组选择布地奈德混悬液及糜蛋白酶雾化吸入;观察组在对照组治疗之外联合以下治疗:三伏第1天和三九第1天分别开始给予健脾补肺汤加减,另外在三伏的每一伏第1天、三九的每一九第1天给予健脾补肺方伏九贴敷。治疗前及治疗12个月后进行自我评估测试问卷(CAT)、肺功能、6 min步行距离(6 MWD)、圣乔治呼吸问卷(SGRQ)、呼吸困难严重程度(mMRC)评价,并进行免疫功能检测,对比治疗前后的各项结果差异。结果 观察组治疗后FVC、FEV1、FEV1/FVC均高于对照组(P<0.05)。观察组治疗后6MWD高于对照组,mMRC评分低于对照组(P<0.05)。观察组治疗后CAT、SGRQ评分均低于对照组(P<0.05)。观察组治疗后CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)及IgA、IgM、IgG水平均高于对照组,CD_(8)^(+)水平低于对照组(P<0.05)。结论 尹莲芳教授应用健脾补肺汤内服配合伏九贴治疗稳定期COPD脾肺两虚证患者可获得满意疗效,患者肺功能、呼吸功能、生活质量及免疫功能均能得到更明显改善,值得推广应用。Objective To study the clinical effect of Professor YIN Lianfang using internal administration of Jianpi Bufei Decoction(健脾补肺汤)in patients with chronic obstructive pulmonary disease(COPD)with deficiency of spleen and lung in stable period.Methods For 140 patients with COPD with deficiency of spleen and lung from January 2020 to September 2020 were divided into two groups,70 cases in each group.In the control group,budesonide suspension and chymotrypsin were inhaled.In addition to the treatment of the control group,the observation group was treated with the following treatments:on the 1 st day of Sanfu and the 1 st day of Sanjiu,Jianpi Bufei Decoction was added,and on the 1 st day of every Fu of Sanfu and the 1 st day of every Jiu of Sanjiu,Jianpi Bufei Decoction was applied.Before treatment and 12 months after treatment,self-assessment test questionnaire(CAT),lung function,6-min walking distance(6 MWD),St.George’s Respiratory Questionnaire(SGRQ)and dyspnea severity(MMRC)were evaluated,and immune function was tested to compare the results before and after treatment.Results After treatment,forced vital capacity(FVC),forced expiratory volume in the first second(FEV1)and FEV1/FVC in the observation group were higher than those in the control group(P<0.05).The 6 MWD of the observation group was higher than that of the control group,and the mMRC score was lower than that of the control group(P<0.05).CAT and SGRQ scores in t heobservation group were lower than those in the control group after treatment(P<0.05).After treatment,the levels of CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+),IgA,IgM and IgG in the observation group were higher than those in the control group,while CD_(8)^(+) level was lower than that in the control group(P<0.05).Conclusion Professor YIN Lianfang’s application of Jianpi Bufei Decoction for oral administration and fumigation combined with Fujiu application in the treatment of stable COPD patients with deficiency of spleen and lung syndrome can obtain satisfactory efficacy.The lung function,resp
