重视穿支皮瓣设计进一步提升我国创面修复水平

摘　　要：皮瓣移植是治疗体表皮肤软组织与器官缺损的主要技术手段, 但传统皮瓣移植的目标大多局限于皮瓣成活和创面愈合, 少有考虑受区功能与外形的恢复和如何减少供区损害。穿支皮瓣新技术、新理念的出现, 特别是特殊形式穿支皮瓣系列术式的衍生与发展, 使得显微重建步入新阶段, 皮瓣设计的重要性越来越受到显微重建外科医生的关注与重视, 穿支皮瓣不仅要重视"点、线、面"的基础设计, 更要重视其平面、层次与嵌合组织内容的优化设计, 旨在实现"移植皮瓣高质量成活、受区恢复良好外形与功能、供区损害最小化"三者完美统一的显微重建新目标。Flap grafting is the main technical means of treating soft tissues and organ defects on the surface of the body,but the goal of traditional flap grafting is mostly limited to the wound healing and survival of the flap,and few considerations are given to the restoration of the function and appearance of the recipient sites and how to reduce the damage of the donor sites.The emergence of new technologies and new concepts of the perforator flaps,especially the derivation and development of the special forms of the perforator flap series,makes the micro-reconstruction into a new stage,the importance of the flap design is more and more concerned by the micro-reconstructive surgeons.In order to apply the perforator flap more efficiently,surgeons are expected not only to pay attention to the basic design of"points,lines and plane",but also to pay attention to the optimal design of its plane,level and chimeric tissue content.It aims to achieve the new goal of perfectly unified micro-reconstruction of"high-quality survival of the grafted flap,good appearance and functional recovery of the recipient sites and minimization of damage to the donor sites".
