摘　　要：目的 分析连续性肾脏替代治疗(CRRT)联合新活素治疗重症监护室(ICU)脓毒症合并心力衰竭的临床疗效.方法 选择2018年10月-2021年12月在广州医科大学附属第五医院治疗的52例ICU脓毒症合并心力衰竭患者作为研究对象,利用随机数字表法分为实验组和对照组,均26例.对照组采用新活素+常规药物辅助治疗,实验组在对照组基础上采用连续性肾脏替代治疗(CRRT).对比两组治疗的效果以及对血清和心功能指标的影响.结果 治疗后,两组LVEF、E/A、MAP高于治疗前,两组HR、血浆黏度低于治疗前(P<0.05);实验组BNP、NT-proBNP低于对照组,CO高于对照组,PCT、IL-6、CRP、全血高切黏度、红细胞比容低于对照组(P<0.05).结论 CRRT联合新活素治疗可有效控制脓毒血症并发心力衰竭患者的机体炎症反应,稳定其血流动力,改善器官功能,值得推广.Objective To analyze the clinical efficacy of continuous renal replacement therapy(CRRT)combined with neoplasms in the treatment of ICU sepsis complicated with heart failure.Methods A total of 52 ICU patients with sepsis complicated with heart failure who were treated in our hospital from October 2018 to December 2021 were selected as the research objects,and were divided into experimental group and control group by random number table method,with 26 cases in both.The control group was treated with neoactin+conventional drug assisted therapy,and the experimental group was treated with continuous renal replacement therapy(CRRT)on the basis of the control group.The effect of treatment and the effect on serum and cardiac function indexes were compared between the two groups.Results After treatment,LVEF,E/A and MAP in both groups were higher than before treatment,while HR and plasma viscosity in both groups were lower than before treatment(P<0.05).BNP and NT-probNP in experimental group were lower than control group,CO was higher than control group,PCT,IL-6,CRP,whole blood high shear viscosity,RBC specific volume were lower than control group(P<0.05).Conclusion CRRT combined with neomycin therapy can effectively control the inflammatory response of sepsis patients with heart failure,stabilize their hemodynamics and improve organ function,which is worthy of promotion.
