妇科腹腔镜术后镇痛治疗中应用不同剂量地佐辛与布托啡诺的疗效观察

摘　　要：目的 探讨妇科腹腔镜术后镇痛治疗中应用不同剂量地佐辛与布托啡诺的效果.方法 选取2020年3月-2021年9月武进中医医院收治的105例行妇科腹腔镜手术的患者为研究对象,将其随机分为三组,分别为A组(低剂量地佐辛)35例、B组(高剂量地佐辛)35例和C组(布托啡诺)35例,比较三组的术后镇痛治疗效果.结果 三组术后4h、8h、24h的血氧饱和度(SpO_(2))、心率(HR)、呼吸率(RR)及平均动脉压(MAP)水平相比,差异无统计学意义(P>0.05).三组术后4h、8h、24h的Ramsay评分和镇痛模拟评分(VAS)相比,差异无统计学意义(P>0.05).A组的术后肛门排气时间明显短于B组和C组(P<0.05);B组和C组的术后肛门排气时间相比,差异无统计学意义(P>0.05).三组的静脉自控镇痛满意度相比,差异无统计学意义(P>0.05).三组的不良反应发生率相比,差异无统计学意义(P>0.05),其中B组的不良反应发生率相对较高.结论 妇科腹腔镜术后镇痛治疗中应用高、低剂量地佐辛与布托啡诺均可获得较好的效果,其中采用低剂量地佐辛治疗的患者术后肛门排气时间短,临床可优先考虑使用.Ob ject ive To investigate the effect of different doses of dizocine and Butorphanol in the analgesic treatment after gynecological laparoscopic.Methods 105 patients with gynecological laparoscopic surgery treated in our hospital from March 2020 to September 2021 were randomly divided into three groups:35 cases in group A(low-dose dizocine),35 cases in group B(high-dose dizocine)and 35 cases in group C(butorphanol).Results There was no significant difference in blood oxygen saturation(SpO_(2)),heart rate(HR),respiratory rate(RR)and mean arterial pressure(MAP)among the three groups at 4h,8h and 24 hours after operation(P>0.05).There was no significant difference in Ramsay score and analgesia simulation score(VAS)at 4h,8h and 24 hours after operation(P>0.05).The postoperative anal exhaust time in group A was significantly shorter than that in group B and C(P<0.05);there was no significant difference in postoperative anal exhaust time between group B and group C(P>0.05).There was no significant difference in the satisfaction of patient-controlled intravenous analgesia among the three groups(P>0.05).There was no significant difference in the incidence of adverse reactions among the three groups(P>0.05),and the incidence of adverse reactions in group B was relatively high.Conc lusion The application of high and low doses of dizocine and Butorphanol in the postoperative analgesia of gynecological laparoscopic can achieve good results.Among them,the postoperative anal exhaust time of low dose dizocine is short,which can be given priority in clinic.
