摘  要:目的研究不同加速度暴露模式下局部脑血氧饱和度(rSO_(2))的变化特点,探讨该生理指标在G引起的意识丧失(G-LOC)预警方面的应用价值。方法13名男性受试者在载人离心机上进行了慢增长率(GOR)模式及模拟空战机动(SACM)模式加速度暴露。加速度暴露时用近红外线光谱技术记录加速度暴露过程中的rSO_(2)变化。结果受试者的GOR1耐力为(4.41±0.76)G,GOR2耐力为(5.87±0.95)G,SACM的G-时间耐力为(119.6±77.4)s。rSO_(2)在加速度暴露开始后随着载荷的增加逐渐降低,与加速度暴露前的rSO_(2)基础值相比,到达GOR1耐力终点、到达GOR2耐力终点前5 s和GOR2耐力终点时rSO_(2)平均值分别减少了8%、9%和11%(P<0.01);到达SACM耐力终点前5 s及到达耐力终点时rSO_(2)平均值分别减少了13.0%和13.7%(P<0.01)。到达rSO_(2)最小值与GOR2耐力终点的时间差t_(1)为(-3.0±11.3)s,到达rSO_(2)最小值与SACM暴露耐力终点的时间差t_(3)为(-1.6±5.1)s。结论rSO_(2)在GOR和SACM两种模式加速度暴露时均随着G值或持续时间的增加而逐渐降低。rSO_(2)最小值不适用于G-LOC预警,可以选择到达耐力终点前某一时间点的rSO_(2)值研究制定G-LOC预警阈值。Objective To study the changes of regional cerebral oxygen saturation(rSO_(2))under different+Gz exposure modes,and to explore the application value of this physiological indicator in the early warning of G-induced loss of consciousness(G-LOC).Methods Thirteen male subjects were exposed to gradual onset rate(GOR)mode and simulated aerial combat maneuver(SACM)mode on the human centrifuge.Near-infrared spectroscopy was used to record the changes of rSO_(2) during+Gz exposure.Results The subjects GOR1 tolerance was(4.41±0.76)G,GOR_(2) tolerance was(5.87±0.95)G,and G-duration tolerance of SACM was(119.6±77.4)s.After+Gz exposure,rSO_(2) gradually decreased with the increase of load.Compared with the basic value of rSO_(2) before+Gz exposure,the mean values of rSO_(2) decreased by 8%,9%and 11%at the end of GOR1 tolerance,5 s before the end of GOR2 tolerance,and the end of GOR2 tolerance,respectively(P<0.01).The mean values of rSO_(2) decreased by 13%and 13.7%5 s before and at the end of SACM tolerance,respectively(P<0.01).The duration(t_(1))between the minimum value of rSO_(2) and the end of GOR2 tolerance was(-3.0±11.3)s and the duration(t_(3))between the minimum value of rSO_(2) and the end of SACM tolerance was(-1.6±5.1)s.Conclusion rSO_(2) decreases gradually when G level of GOR or duration of SACM exposure increases.Instead of the minimum value,the value of rSO_(2) at some point before the end of G tolerance is suitable for establishing an early warning threshold value of G-LOC.
