摘  要:目的探讨穿心莲内酯通过介导IL-23/Th17平衡对肺炎克雷伯菌所致重症肺炎大鼠免疫功能及肺部损伤的影响及作用机制。方法建立肺炎克雷伯菌所致重症肺炎大鼠模型,随机分为模型组、穿心莲内酯低、高剂量组及阳性对照组,另设健康组。经一系列干预后,检测大鼠肺功能指标变化;脱颈处死大鼠,取双侧肺组织测量肺湿干质量比;ELISA检测大鼠血清IL-23、IL-17水平;HE染色观察大鼠肺病理学;流式细胞术检测外周血中Th17细胞比例;Western blot检测大鼠肺组织核转录因子kappaB(NF-κB)信号通路相关蛋白表达。结果模型组、穿心莲内酯低、高剂量组、阳性对照组、健康组大鼠静息通气量、气道阻力比值依次升高,肺湿干质量比与血清IL-23、IL-17水平依次降低(P<0.05)。模型组肺泡结构破坏,肺泡腔变实,可见大量炎性细胞浸润与浆液性渗出;与模型组比较,穿心莲内酯低、高剂量组及阳性对照组的肺泡结构完整性、形态均改善,炎性细胞浸润减轻。模型组、穿心莲内酯低、高剂量组、阳性对照组、健康组,大鼠外周血Th17细胞比例依次降低(P<0.05)。模型组、穿心莲内酯低、高剂量组、健康组,大鼠肺组织p-IκBα蛋白表达水平依次升高,p-NF-κB蛋白表达水平依次降低(P<0.05)。结论穿心莲内酯可通过介导IL-23/Th17平衡,增强肺炎克雷伯菌所致重症肺炎大鼠免疫功能,减轻肺部损伤,这可能与调节IκBα/NF-κB通路有关。Objective To investigate the effect and mechanism of andrographolide on immune function and lung injury in rats with severe pneumonia induced by Klebsiella pneumoniae through mediating IL-23/Th17 balance.Methods Rat model of severe pneumonia caused by Klebsiella pneumoniae was established.The rats were randomly divided into model group,andrographolide low-dose group,andrographolide high-dose group and positive control group.Another health group was set up.The changes of lung function indexes were detected after a series of dry prognosis.The weight ratio of wet to dry of bilateral lung was measured after the rats were sacrificed by neck removal.The serum levels of IL-23 and IL-17 were detected by ELISA.The lung pathology was observed by HE staining.The proportion of Th17 cells in peripheral blood was detected by flow cytometry.The protein expression of nuclear factor kappa B(NF-κB)signaling pathway was detected by Western blot in rat lung tissue.Results In model group,andrographolide low-dose group,andrographolide high-dose group,andrographolide positive control group and healthy group,the resting ventilation volume and airway resistance ratio increased in turn,while the lung weight ratio of wet to dry,and serum IL-23 and IL-17 levels decreased in turn(P<0.05).In the model group,the alveolar structure was destroyed,the alveolar cavity became solid,and a large number of inflammatory cell infiltration and serous exudation could be observed.Compared with the model group,the integrity and morphology of alveolar structure were improved,and the infiltration of inflammatory cells was reduced in the low and high dose andrographolide groups and the positive control group.The proportion of Th17 cells in peripheral blood of rats in model group,andrographolide low-dose group,andrographolide high-dose group,positive control group and healthy group decreased in turn(P<0.05).The protein expression level of p-IκBαin lung tissue of rats in model group,andrographolide low-dose group,high-dose group,healthy group,increased in tur
