摘 要:目的探讨聚乳酸羟基乙酸共聚物(PLGA)支架复合骨髓间充质干细胞(BMSCs)修复半月板缺损的效果。方法提取兔BMSCs并体外扩增,载入PLGA支架。12只兔随机分为3组,构建单侧膝关节半月板缺损模型,分别对缺损采取旷置(模型组)、植入PLGA支架(PLGA组)或PLGA/BMSCs(PLGA/BMSCs组)的处理,以对侧未造损膝关节的半月板为对照组。术后12周采集半月板样本,进行大体观察、二甲基亚甲基蓝(DMMB)染色、Ⅱ型胶原免疫组织化学染色及酶联免疫吸附试验(ELISA),以评估半月板修复效果。结果与模型组比较,PLGA组和PLGA/BMSCs组在大体观察中显示出更饱满的缺损填充;该2组的修复组织在DMMB染色中也显现更高的蛋白多糖含量,尤其是PLGA/BMSCs组的蛋白多糖填充更为致密。3个干预组修复组织的Ⅱ型胶原免疫组织化学染色强度均高于对照组。ELISA显示,PLGA/BMSCs组的Ⅱ型胶原表达(5.216±0.345)均显著高于对照组(4.409±0.284)和PLGA组(4.498±0.452,F=7.093,P<0.05)。结论在半月板缺损植入PLGA支架可以促进修复组织填充缺损;PLGA支架可以支持BMSCs的纤维软骨分化,而植入PLGA/BMSCs构建体较PLGA支架更加有助于进一步增进半月板修复组织中蛋白聚糖和Ⅱ型胶原组分的生成。Objective To investigate the effect of polylactic acid-co-glycolic acid(PLGA)scaffold combined with bone marrow mesenchymal stem cells(BMSCs)on meniscus defect repair.Methods Rabbit BMSCs were extracted and amplified in vitro and loaded into PLGA scaffolds.Twelve rabbits were randomly divided into 3 groups to establish a unilateral knee meniscus punch defect model.The defects were treated with open placement(model group),PLGA scaffold(PLGA group)or PLGA/BMSCs(PLGA/BMSCs group),and the meniscus without knee joint injury was used as control group.Meniscal samples were collected 12 weeks after surgery for gross observation,dimethylmethylene blue(DMMB)staining,immunohistochemical staining for typeⅡcollagen and enzyme-linked immunosorbent assay(ELISA)to evaluate the effectiveness of meniscal repair.Results The PLGA group and the PLGA/BMSCs group showed fuller defect filling in the gross observation compared with the model group.The repaired tissues of the two groups also showed higher proteoglycan content in DMMB staining,especially in the PLGA/BMSCs group.The immunohistochemical staining intensity of typeⅡcollagen in the repaired tissues of the three intervention groups was higher than that of the control group.ELISA showed that the expression of typeⅡcollagen(5.216±0.345)in PLGA/BMSCs group was significantly higher than that in control group(4.409±0.284)and PLGA group(4.498±0.452,F=7.093,P<0.05).Conclusion Implantation of PLGA scaffolds in meniscal defects can promote the filling of repaired tissues.PLGA scaffolds can support the fibrocartilage differentiation of BMSCs,and implantation of PLGA/BMSCs constructs is more helpful than PLGA scaffolds to further enhance the production of proteoglycan and typeⅡcollagen components in meniscal repaired tissues.
