摘 要:目的探讨坐骨神经慢性卡压损伤对大鼠腰椎4~6(L4~6)脊髓节段内基质金属蛋白酶-9(MMP-9)和金属蛋白酶组织抑制剂-1(TIMP-1)的表达变化及意义。方法2021年7月至2021年12月,将120只SD大鼠(华中科技大学实验动物中心提供)采用抽签法分成A、B两组。A组仅分离暴露坐骨神经,为假手术组;B组对SD大鼠右侧后腿坐骨神经造成慢性卡压损伤,为卡压手术组。于造模成功后1、2、3、4、5个月时间点随机取各组大鼠12只处死;6只灌注后逆行追踪坐骨神经至脊髓,截取大鼠坐骨神经对应L4~6节段脊髓行形态学及免疫组织化学观察;6只直接取对应L4~6节段脊髓行RT-PCR、蛋白质印迹法(Western blot)定量测定MMP-9及TIMP-1含量。组间比较采用独立样本t检验。结果B组大鼠L4~6脊髓节段内神经元数目、髓鞘面积及占比低于A组(314.63±43.56比352.27±33.74,68.61±5.36比75.61±6.57,23.74±3.37比26.82±3.48,t=2.351、3.622、3.856,P<0.05);B组大鼠L4~6脊髓节段内MMP-9及TIMP-1含量高于A组(23.96±1.22比19.96±1.37,t=4.847,P<0.05),(19.49±1.26比14.57±1.22,t=3.681,P<0.05);MMP-9 mRNA及蛋白在3月时达峰值(5.96±1.03比4.69±0.72,t=3.477,P<0.05),(64.55±4.85比55.83±4.57,t=4.278,P<0.05),TIMP-1在4月时达峰值(4.04±0.61比2.71±0.41,t=3.178,P<0.05),(52.41±2.96比39.58±3.21,t=3.471,P<0.05);TIMP-1的高表达滞后于MMP-9的表达。结论坐骨神经慢性卡压损伤可导致对应脊髓节段内MMP-9及TIMP-1表达的升高,表明MMP-9及TIMP-1在脊髓神经元退变和死亡过程中起到一定作用。Objective To investigate the expression and effects of matrix metalloproteinase-9(MMP-9)and Tissue inhibitor of metalloproteinase-1(TIMP-1)in the related Lumbar spinal cord segments(L4-6)after the chronic sciatic nerve compression injury of rats.Methods From July 2021 to December 2021,120 adult male SD rats(Provided by Huazhong University of Science and Technology Laboratory Animal Center)were randomly divided into group A and B:group A(sham-operated group):only exposed the sciatic nerve;group B(compression group):undergone sciatic nerve entrapment operation on the right hind leg.At the time of 1,2,3,4 and 5 months after the surgery,12 rats were randomly selected from group A and B respectively and sacrificed.The sciatic nerve of 6 rats was followed retrograde to the spinal cord after perfusion,and the spinal cord segments of L4-6 were intercepted for the detection of morphological and immunohistochemical.RT-PCR and Western blotting of MMP-9 and TIMP-1 were observed in the L4-6 spinal cord segment without perfusion.Independent sample t test was used for comparison of measurement data of the two groups in accordance with normal distribution.Results The number of neurons,myelin sheath area and proportion in L4-6 spinal cord segment of rats in group B were lower than those in group A(314.63±43.56 vs.352.27±33.74,68.61±5.36 vs.75.61±6.57,23.74±3.37 vs.26.82±3.48,t=2.351,3.622,3.856,P<0.05).The content of group B within L4-6 rats spinal cord segmental MMP-9 and TIMP-1 were significantly higher than group A(23.96±1.22 vs.19.96±1.37,t=4.847,P<0.05),(19.49±1.26 vs.14.57±1.22,t=3.681,P<0.05).MMP-9 reached its peak in 3 months(5.96±1.03 vs.4.69±0.72,t=3.477,P<0.05),(64.55±4.85 vs.55.83±4.57,t=4.278,P<0.05),and TIMP-1 reached its peak in 4 months(52.41±2.96 vs.39.58±3.21,t=3.471,P<0.05).The high expression of TIMP-1 lagged behind that of MMP-9.Conclusion Chronic entrapment injury of sciatic nerve can lead to the increase of MMP-9 and TIMP-1 expression in corresponding spinal cord segments,indicating that MMP
