摘 要:目的观察电针心肺经五输穴对急性心肌缺血(AMI)的影响,并探讨其作用机制。方法将108只SD大鼠随机分为假手术组、模型组,心经组:少冲、少府、神门、灵道、少海组,肺经组:少商、鱼际、太渊、经渠、尺泽组,每组9只。采用冠状动脉左前降支结扎法建立AMI模型。造模后各组电针相应穴位,每日1次,每次30 min,共3 d。HE染色法观察心肌组织病理形态学改变。Elisa法检测血清内皮型一氧化氮合酶(eNOS)、总抗氧化能力(T-AOC)、降钙素基因相关肽(CGRP)水平,TUNEL法检测计算心肌细胞凋亡(AI)指数,Western Blot测量心肌组织凋亡相关蛋白B细胞淋巴瘤-2蛋白(Bcl-2)、Bcl-2腺病毒E/B19 kD相互作用蛋白(BNIP3)、半胱氨酸天冬氨酸蛋白酶(Caspase-3)、Bcl-2关联X蛋白质(BAX)表达。结果与假手术组比较,模型组心肌纤维排列紊乱、断裂,细胞肿胀破裂渗血,AI指数,心肌组织BNIP3、Caspase-3、BAX蛋白表达及BAX/BCL-2比值升高(P<0.01),血清e NOS、T-AOC、CGRP水平、心肌组织蛋白BCL-2表达降低(P<0.01)。与模型组比较,心经各组心肌纤维紊乱有所恢复,细胞破裂渗血状况改善明显,心经各组血清eNOS、CGRP水平,心经神门、灵道、少海组血清T-AOC水平,心经少冲、少府、神门组心肌组织BCL-2蛋白表达上升(P<0.05,P<0.01),心经各组心肌组织BNIP3蛋白表达,心经少冲、少府、神门组Caspase-3、BAX表达及BAX/BCL-2比值,心经少府、神门、少海组AI指数下降(P<0.05,P<0.01);肺经组心肌纤维排列和细胞破裂渗血情况未明显好转,肺经太渊组血清CGRP,肺经尺泽组血清eNOS、T-AOC、CGRP水平,肺经鱼际组BCL-2表达升高(P<0.05,P<0.01),肺经鱼际组BAX表达及BAX/BCL-2比值,肺经太渊组Caspase-3表达下降(P<0.05)。与肺经少商组比较,心经少冲组血清eNOS、CGRP水平及BCL-2表达上升;与肺经鱼际组比较,心经少府组血清eNOS水平上升(P<0.05);与肺经太渊组比较,心经神门组AI指数及BNObjective To observe the effect of electroacupuncture(EA)at Wushu points of Xin meridian and Fei meridian on acute myocardial ischemia(AMI)and explore its mechanism.Methods Totally 108 SD rats were randomly divided into sham group,model group,Xin meridian groups:Shaochong(HT9),Shaofu(HT8),Shenmen(HT7),Lingdao(HT4),Shaohai(HT3)group,Fei meridian groups:Shaoshang(LU11),Yuji(LU10),Taiyuan(LU9),Jingqu(LU8)and Chize(LU5)group,with 9 rats in each group.AMI model was established by ligation of left anterior descending coronary artery.After modeling,EA at corresponding acupoints in each group was performed continuously 3 days,once a day,30 minutes each time.The pathological changes of myocardial tissue were observed by HE staining.Elisa was used to determine the level of endothelial nitric oxide synthase(eNOS),total antioxidant capacity(T-AOC)and calcitonin gene-related peptide(CGRP)in serum.TUNEL method was used to determine the myocardial apoptosis index(AI).Western Blot was used to measure the expression of myocardial apoptosis-related proteins B cell lymphoma-2 protein(BCL-2),Bcl-2 adenovirus E/B19 kD interaction protein(BNIP3),Caspase-3,and Bcl-2-associated X protein(BAX).Results Compared with the sham group,the myocardial fibers in the model group were disorganized and fractured,and the cells were swollen,ruptured and bleeding.The AI,expressions of BNIP3,Caspase-3and BAX in myocardial tissue and BAX/BCL-2 increased(P<0.01),serum eNOS,T-AOC,and CGRP and expression of BCL-2 in myocardial tissue decreased in model group(P<0.01).Compared with model group,the disorder of myocardial fibers in Xin meridian groups was recovered,and the state of cell rupture and bleeding was obviously improved.Serum eNOS and CGRP in Xin meridian groups,serum T-AOC in HT7,HT4and HT3 group,expression of BCL-2 in myocardial tissues in HT9,HT8 and HT7 group increased(P<0.05,P<0.01).The expression of BNIP3 in myocardial tissues in Xin meridian groups,expression of Caspase-3,BAX and BAX/BCL-2 in HT9,HT8,and HT7 group,AI index in HT8,HT7 and HT3 g
