睡眠呼吸暂停低通气综合征与高血压视网膜病变的关系探究

摘　　要：目的探讨睡眠呼吸暂停低通气综合征与高血压视网膜病变的关系.方法选取2020年7月-2022年3月收治并确诊的睡眠呼吸暂停低通气综合征(OSAHS)患者100例,根据病情程度分为轻、中、重三组.三组均行眼底检查视网膜病变,观察三组患者视网膜病变严重程度,作统计学分析,观察三组差异是否具有统计学意义.结果35例纳入轻度组,38例纳入中度组,27例纳入重度组.两组患者在年龄、性别和病程方面差异并无统计学意义(P>0.05).重度组患者4%氧减饱和度指数、睡眠障碍性呼吸百分比要高于轻度组和中度组患者,平均动脉血氧饱和度低于轻度组和中度组患者(P<0.05).随着OSAHS病情严重程度的增加,高血压性视网膜病变3/4级严重病变的患者比例升高(P<0.05).结论OSAHS会导致患者夜间的低氧以及再氧合化,并对患者的舒张压与收缩压产生影响,导致高血压视网膜病变的发生与发展.对于OSAHS应引起足够重视,尽早给予防治措施.Objective To investigate the relationship between sleep apnea hypopnea syndrome and hypertensive retinopathy.Methods A total of 100 patients with sleep apnea hypopnea syndrome(OSAHS)admitted from July 2020 to March 2022 were selected and divided into mild,moderate and severe groups according to the severity of the disease.Retinal fundus examination was performed in all three groups,and the retinas of patients in the three groups were observed.The severity of the disease was analyzed statistically,and the difference between the three groups was statistically significant.Results 35 cases were included in mild group,38 cases in moderate group and 27 cases in severe group.There were no significant differences in age,gender and course of disease between 2 groups(P>0.05).4%oxygen desaturation index and percentage of sleep-disordered breathing in severe group were higher than those in mild and moderate groups,and mean spo2 was lower than those in mild and moderate groups(P<0.05).With the increase of OSAHS severity,the proportion of patients with grade 3/4 severe hypertensive retinopathy increased(P<0.05).Conc lus ion OSAHS can cause hypoxia and reoxygenation in patients at night,and affect the diastolic and systolic blood pressure of patients,leading to the occurrence and development of hypertensive retinopathy.Sufficient attention should be paid to OSAHS and prevention measures should be taken as soon as possible.
