胸腔积液IGRA试验对老年结核性胸膜炎的诊断价值

摘　　要：目的 探究胸腔积液胸水γ干扰素释放(IGRA)试验应用于老年结核性胸膜炎中的诊断价值.方法 选取湖南省长沙市第一医院2020年3月-2022年3月收治的结核性胸膜炎老年患者(64例)为探究对象,将入选患者分成胸腔组与氨酶组,每组各32例,分别采集两组患者的胸水,以临床诊断为标准,分别对两组患者采取不同的检测方式,其中氨酶组行胸水腺苷脱氨酸(ADA)检测,胸腔组行胸腔积液IGRA检测,观察两组患者的阳性、阴性、假阳性、假阴性、准确性、阳性预测值、阴性预测值、灵敏度、特异度.结果 以临床诊断作为金标准,胸腔组的阳性率、假阳性率、阴性率、假阴性率与氨酶组相比,两组差异不具有统计学意义(P>0.05).与氨酶组相比,胸腔组的特异度、灵敏度、阳性预测值、阴性预测值均要更高,而误诊漏诊率显著低于氨酶组,差异具有统计学意义(P<0.05).结论 胸腔积液IGRA试验应用于老年结核性胸膜炎中具有较好的诊断价值.Objective To explore the diagnostic value of pleural effusion interferon release(IGRA)in elderly tuberculous pleurisy.Methods Selected in March 2020 to March 2022,tuberculous pleurisy elderly patients(64cases)for the object,will be selected into pleural group and aminase group,each group 32 cases,collect two groups of pleural fluid,diagnosis as clinical standard,adopt different detection methods for two groups of patients,including aminase group thymic adenosine deamine(ADA)test,pleural effusion IGRA test,observe positive,negative,false negative,false negative,two groups,accuracy,positive predictive value,negative predictive value,sensitivity,specificity.Results Clinical diagnosis as the gold standard,the true positive rate,false negative rate,negative rate and false negative rate in the thoracic group were not statistically significant compared with the ammonia enzyme group(P>0.05).Compared with the ammonia enzyme group,the specificity,sensitivity,positive predictive value and negative predictive value of the thoracic group were higher,while the misdiagnosis and missed diagnosis rate was significantly lower than that of the ammonia enzyme group(P<0.05).Conc lus ion Pleural effusion IGRA experiment has good diagnostic value in elderly tuberculous pleurisy.
