髋臼结构性植骨联合人工全髋关节置换术在成人发育性髋关节发育不良中的应用

摘　　要：目的 探讨成人发育性髋关节发育不良(DDH)患者给予髋臼结构性植骨联合人工全髋关节置换术(THA)治疗的效果.方法 选取2019年7月-2021年8月江西省彭泽县人民医院收治的成人DDH患者共计64例,分析患者手术相关指标、影像学检查结果、髋关节功能及术后并发症.结果 患者手术时间在120-180min,平均手术时间(150.46±18.37)min,术中出血量在305-865mL,平均术中出血量(580.43±105.34)mL,术后引流量为285-755mL,平均术后引流量(450.78±101.42)mL;术后双下肢长度差较术前明显更短(P<0.05),髋关节活动度明显更大(P<0.05);术后Harris评分较术前明显更高(P<0.05);64例患者中4例(6.25%)患者出现术后并发症.结论 成人DDH患者给予髋臼结构性植骨联合THA治疗的手术效果较好,能够改善影像学检查结果及髋关节功能,促进患者早日恢复正常生活.Objective To investigate the effect of acetabular structural bone grafting combined with total hip replacement(THA)on adult developmental dysplasia of the hip(DDH).Methods A total of 64 adult DDH patients treated in our hospital from July 2019 to August 2021 were selected,and the related indicators of surgery,imaging results,hip function and postoperative complications were analyzed.ResulI ts The operation time was 120-180min,the average operation time was(150.46±18.37)min,the intraoperative bleeding volume was 305-865ml,the average intraoperative bleeding volume was(580.43±105.34)ml,and the postoperative drainage volume was 285-755ml,the average postoperative drainage volume was(450.78±101.42)ml;the length difference of both lower limbs after operation was significantly shorter than that before operation(P<0.05),and the range of motion of hip joint was significantly greater(P<0.05);harris score after operation was significantly higher than that before operation(P<0.05);among the 64 patients,4(6.25%)had postoperative complications.Conclusion The surgical effect of acetabular structural bone grafting combined with THA in adult patients with DDH is better,which can improve the imaging results and hip function,and promote the patients to return to normal life as soon as possible.
