针刺配合自拟益气复律汤内服对室性心律失常患者QT离散度的影响

摘　　要：目的:研究室性心律失常患者使用针刺配合自拟益气复律汤内服方案治疗疗效及对患者QT离散度的影响,以分析针刺配合自拟益气复律汤内服方案是否治疗室性心律失常有效。方法:选取184例室性心律失常患者,分为对照组92例、观察组92例,均根据疾病专家共识中推荐方案采用常规治疗措施干预,对照组内服自拟益气复律汤,观察组在对照组的基础上配合针刺。比较两组心功能、心率变异性、QT离散度、环磷酸腺苷(cAMP)、蛋白激酶A(PKA)蛋白表达量、中医证候积分及临床疗效。结果:两组治疗前心功能、心率变异性、QT离散度、cAMP、PKA蛋白表达量、中医证候积分比较,差异无统计学意义(P>0.05)。两组治疗前心功能、心率变异性、QT离散度、cAMP、PKA蛋白表达量、中医证候积分均较治疗前降低,差异有统计学意义(P<0.05)。治疗后观察组心功能、心率变异性、QT离散度、cAMP、PKA蛋白表达量、中医证候积分均显著低于对照组,差异有统计学意义(P<0.05)。观察组总有效率显著高于对照组,差异有统计学意义(P<0.05)。结论:针刺配合自拟益气复律汤内服可明显改善室性心律失常心功能、心率变异性,降低QT离散度,其可能经降低cAMP、PKA蛋白表达发挥作用。Objective To study the efficacy of acupuncture with acupuncture and the effect on QT dispersion in patients,in order to analyze whether the internal regimen of acupuncture with self-planned qi rhythm soup is effective in treating ventricular arrhythmia.Method 184 patients with ventricular arrhythmia were selected and divided into control group 92 cases and observation group 92 cases.According to the scheme recommended in the consensus of disease experts,routine treatment measures were adopted for intervention.The control group took Yiqi Fulu decoction,and the observation group cooperated with acupuncture on the basis of the control group.Cardiac function,heart rate variability,QT dispersion,camp,PKA protein expression,TCM syndrome score and clinical efficacy were compared between the two groups.Results There was no significant difference in cardiac function,heart rate variability,QT dispersion,camp,PKA protein expression and TCM syndrome score between the two groups before treatment(P>0.05).The cardiac function,heart rate variability,QT dispersion,camp,PKA protein expression and TCM syndrome score of the two groups before treatment were significantly lower than those before treatment(P<0.05).After treatment,the cardiac function,heart rate variability,QT dispersion,cAMP,PKA protein expression,and TCM syndrome scores in the observation group were significantly lower than those in the control group,with statistical differences(P<0.05).The total effective rate in the observation group was significantly higher than that in the control group,with statistical difference(P<0.05).Conclusion Acupuncture combined with oral administration of self-made Yiqi Fulu decoction can significantly improve ventricular arrhythmia cardiac function,heart rate variability,and reduce QT dispersion,which may play a role by reducing the expression of cAMP and PKA proteins.
