摘　　要：目的 探索行之有效的住院患者不良事件监测方法。方法 利用医院信息管理系统随机抽取上海市嘉定区中医医院住院病历进行回顾性分析,采用美国医疗改进中心全过程触发器(IHI-GTT)检测住院患者不良事件,评价病历中的不良事件发生率。结果 抽查100份住院病历,共18项触发器阳性,阳性频次为147次。确定不良事件23件,涉及14例患者。每100例患者不良事件数为23件,发生不良事件患者比例为14.0%,每1000床日不良事件数为17.2件。结论 IHI-GTT能在住院患者中有效检出不良事件,值得持续使用本工具监测不良事件发生率并持续改进。Objective To explore an effectual method for monitoring adverse event(ADE) in inpatients. Methods Inpatient medical records of Jiading Hospital of Traditional Chinese Medicine in Shanghai were randomly selected by using the hospital information system for retrospective analysis. The ADE in inpatients were monitored with the institute for healthcare improvement-global trigger tool(IHI-GTT), and the incidence of ADE in medical records was evaluated. Results A total of 18 positive triggers were found in 100 inpatient medical records screened, with a positive frequency of 147. 23 ADE were identified, involving 14 patients. 23 ADE per 100 patients were observed, the proportion of patients with ADE was 14.0%, and the number of ADE per day for every 1000 beds was 17.2. Conclusion The IHI-GTT can effectively detect ADE in inpatients, and therefore it is worthwhile to continuously monitor the incidence of ADE and make continuous improvement with the help of this tool.
