宫颈扩张球囊在瘢痕子宫足月妊娠阴道试产中的应用效果

摘　　要：目的:观察宫颈扩张球囊在瘢痕子宫足月妊娠阴道试产中的应用效果。方法:选取2018年1月至2020年12月该院收治的100例瘢痕子宫足月妊娠阴道试产产妇进行前瞻性研究,按照随机数字表法分为研究组与对照组各50例。对照组予以常规静脉滴注缩宫素诱导分娩,研究组采用宫颈扩张球囊诱导分娩,比较两组诱导分娩前和诱导分娩12 h Bishop评分,诱导分娩12 h宫颈成熟率,分娩方式和新生儿出生后5 min、10 min Apgar评分。结果:诱导分娩12 h,两组Bishop评分均高于诱导分娩前,且研究组高于对照组,差异有统计学意义(P<0.05);诱导分娩12 h,研究组宫颈成熟率为84.00%,明显高于对照组的66.00%,差异有统计学意义(P<0.05);研究组阴道分娩率为92.00%,明显高于对照组的52.00%,差异有统计学意义(P<0.05);研究组剖宫产率为8.00%,明显低于对照组的48.00%,差异有统计学意义(P<0.05);研究组新生儿出生后5 min、10 min Apgar评分均高于对照组,差异有统计学意义(P<0.05)。结论:宫颈扩张球囊应用于瘢痕子宫足月妊娠阴道试产产妇可提高Bishop评分、宫颈成熟率、阴道分娩率和新生儿Apgar评分,降低剖宫产率,效果优于常规静脉滴注缩宫素。Objective:To observe application effects of cervical dilatation balloon in full-term vaginal trial delivery women with scar uterus.Methods:100 full-term vaginal trial delivery women with scar uterus admitted to the hospital from January 2018 to December 2020 were selected as the research objects.They were divided into study group and control group according to the random number table method,50 cases in each.The control group was given routine intravenous infusion of Oxytocin to induce delivery,while the study group was induced by cervical dilatation balloon.The Bishop scores before induction delivery and 12 h after induction delivery,the cervical ripening rate 12 h after induction delivery,the delivery mode and the Apgar scores 5 min and 10 min after birth were compared between the two groups.Results:12 h after induced delivery,the Bishop scores of the two groups were higher than those before induced delivery,that of the study group was higher than that of the control group,and the differences were statistically significant(P<0.05).12 h after induced delivery,the cervical ripening rate of the study group was 84.00%,which was significantly higher than 66.00%of the control group,and the difference was statistically significant(P<0.05).The vaginal delivery rate of the study group was 92.00%,which was significantly higher than 52.00%of the control group,and the difference was statistically significant(P<0.05).The cesarean section rate of the study group was 8.00%,which was significantly lower than 48.00%of the control group,and the difference was statistically significant(P<0.05).Further,the Apgar scores at 5 min and 10 min after birth in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:Cervical dilatation balloon can improve the Bishop score,the cervical ripening rate,the vaginal delivery rate and the neonatal Apgar score,and reduce the cesarean section rate of the full-term vaginal delivery women with scar uterus.Moreover,it is superi
