类风湿关节炎患者相关实验室指标与病情活动度的关系

摘　　要：目的探讨类风湿关节炎(RA)患者相关实验室指标与病情活动度的关系。方法回顾性选取2018年1月至2020年12月收治的RA患者130例为RA组,另选60例同期进行健康体检的健康人群为对照组。将RA组患者按疾病活动指数(DAS28)分为高活动期组(DAS28>5.1分,65例)和低活动期组(DAS282.6~3.2分,65例)。比较RA组、对照组血清三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白(LDL)及高密度脂蛋白(HDL)水平,低活动期组、高活动期组及对照组全血红细胞体积分布(RDW)、炎性因子指标水平,Pearson相关性分析全血RDW、血清抗CCP抗体、RF-IgG、RF-IgM及RF-IgA水平与全血ESR、血清CRP水平的关系。结果RA组血清TG、TC水平高于对照组,血清HDL水平低于对照组(P<0.05)。高活动期组全血RDW、ESR、血清CRP、抗CCP抗体、RF-IgG、RF-IgM及RF-IgA水平均显著高于低活动期组和对照组(P<0.05);低活动期组全血RDW、ESR、血清CRP、抗CCP抗体、RF-IgG、RF-IgM及RF-IgA水平均显著高于对照组(P<0.05)。Pearson分析结果显示,全血RDW、血清抗CCP抗体、RF-IgG、RF-IgM及RF-IgA水平与全血ESR、血清CRP水平均呈正相关(r=0.491、0.472;0.483、0.648;0.425、0.523;0.526、0.484;0.491、0.476,P<0.05)。结论RA患者相关实验室指标变化与患者病情活动度密切相关,可作为RA患者病情活动度判断的重要指标。Objective To explore the relationship between rheumatoid arthriti(RA)-related laboratory indicators and disease activity.Methods Totally 130 RA patients who visited our hospital were retrospectively enrolled as the RA group,and 60 current health examination people as the control group.The RA group was further assigned to high activity group[disease activity score in 28 joints(DAS28)>5.1 points,65 cases]and low activity group(DAS282.6~3.2 points,65 cases).The intergroup triglycerides(TG),total cholesterol(TC),low density lipoprotein(LDL),high density lipoprotein(HDL),red cell distribution width(RDW),inflammatory markers were measured.The correlation between RDW,anti-CCP antibody,rheumatoid factor(RF)-immunoglobulin G(IgG),RF-IgM,RF-IgA and erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)was assessed by Pearson correlation analysis.Results RA group had significant higher TG and TC and significant lower HDL than the control group(P<0.05).The high activity group had significant higher RDW,ESR,CRP,anti-CCP antibodies,RF-IgG,RF-IgM and RF-IgA than the low activity group and the control group(P<0.05);in the low activity group and the control group,but superior in the low activity group(P<0.05).Pearson analysis indicates that the levels of RDW,anti-CCP antibody,RF-IgG,RF-IgM and RF-IgA were positively correlated with ESR and CRP(r values were 0.491,0.472;0.483,0.648;0.425,0.523;0.526,0.484;0.491,0.476,respectively,P<0.05).Conclusion RA-related laboratory indicators are closely related to disease activity,and may be considered as an important indicator for the judgment of the disease activity in RA patients.
